胰母细胞瘤的CT表现(2例报道并文献复习)

目的探讨胰母细胞瘤的CT表现,提高对该病的CT诊断水平。方法回顾性分析2例均行CT动态增强扫描并经手术病理证实的胰母细胞瘤的CT表现,并与病理结果对照,总结胰母细胞瘤CT表现特点。结果 2例患者均为婴儿,以皮肤粘膜黄染为主要体征而入院治疗。CT均表现为胰头软组织肿块,边界较清楚,密度不均匀,肿块内均可见沙粒状钙化,伴胰管和/或肝内外胆管轻度扩张,并包绕邻近肠系膜血管和门静脉。1例肿块侵犯邻近十二指肠肠壁。CT增强后,1例表现为较均匀轻度强化,另1例表现为明显不均匀强化。结论胰母细胞瘤CT表现较具特征性,结合临床表现及好发年龄可在术前作出诊断。

Imaging features of pancreatoblastom:2 cases report and review of the literature

Objective To investigate the imaging features ofpancreatoblastoma and to improve the diagnostic confidence for this disease. Methods Unenhanced and dynamic contrast-enhanced CT was performed on 2 patients with surgically and pathologically proven pancreatoblastoma. The clinical characteristics and CT findings were analyzed retrospectively. Results Both patients were infants presenting with jaundice. Large, well-circumscribed, heterogeneous mass was noted in the head of pancreas with punctate calcifications, mild pancreatic duct, intrahepatic and extra-hepatic bile duct dilatation surrounding the mesenteric vessels and portal vein. There was tumor invading the duodenum in both patients. The tumor enhanced heterogeneously in one patient and only slightly in the other infant. Conclusion CT findings of pancreatoblastoma are characteristic. Combining with the clinical feature and age of onset, the diagnosis can be preoperatively suggested.
