后腹腔镜与开放手术行肾及肾上腺肿瘤切除临床对比研究

目的探讨经后腹腔镜行肾及肾上腺肿瘤切除术的临床疗效及安全性。方法回顾性分析2005年10月至2013年12月253例肾及肾上腺肿瘤切除术患者临床资料,其中后腹腔镜组125例,开放手术组128例,比较两组手术时间、术中出血量、术后疼痛强度、术后住院时间情况。结果后腹腔镜组122例手术获得成功,3例中转开放手术。后腹腔镜组手术时间、术中出血量、术后疼痛强度、术后住院时间与开放手术组比较差异均有统计学意义(P<0.05)。结论后腹腔镜手术治疗肾及肾上腺肿瘤具有创伤小、并发症少、术后恢复快、住院时间短等优势,疗效满意,安全性及应用价值高,值得在基层医院推广应用。

Comparison of clinical efficacy in kidney and adrenal tumor resection by retroperitoneal ;laparoscopic and open surgery

Objective To investigate the retroperitoneoscopy surgery clinical curative effect and safety of renal and adrenal tumors, and sum up experience. Methods Retrospective analysis conducted in October 2005 to December 2013 of the 253 cases of kidney and adrenal gland tumor resection, 125 cases of laparoscopic group, 128 of open surgery, compared with two groups of intraoperative blood loss, operative time, postoperative pain, postoperative hospital stay. Results 122 cases of surgical success and turn to open in 3 of retroperitoneoscopy group, operation time, intraoperative blood loss, postoperative pain intensity, postoperative hospital stay compared with open surgery group were statistically significant （P＆lt;0.05）. Conclusions Retroperitoneal laparoscopic surgery treatment of kidney and adrenal tumor has small trauma, fewer complications, quicker recovery, shorter hospitalization time and other advantages, curative effect satisfaction, safety and high application value, worthy of popularization and application in grassroots hospital.
