老年妇科疾病前后10年临床分析

为了探讨老年妇科疾病临床特征及前后 10年疾病种类的变化 ,为开展老年妇科疾病的防治工作提供参考 ,分析 1980年 1月～ 1999年 12月 2 0年中 2 44例住院老年妇科病人的疾病及合并症、并发症 ,并对前后 10年两组进行比较。结果 :资料所示老年妇科疾病的发病数以生殖道肿瘤最多占 6 1.0 7% ,生殖道陈旧性创伤性疾病占 2 6 .6 4% ,非肿瘤性异常子宫出血性疾病占 6 .15 % ,生殖道炎症性疾病占 4.92 % ,其他占 1.2 3%。恶性肿瘤占全部老年妇女患者的18.0 3% ,其中前 10年卵巢癌占首位 ,后 10年子宫内膜癌占首位。后 10年恶性肿瘤发病率减少 (P<0 .0 1)。术前合并症中 ,前 10年呼吸系统疾病发病率高 (P<0 .0 1) ,后 10年高血脂症发病率高 (P<0 .0 5 )。其他均无显著差别。老年妇科疾病种类虽多 ,发病率升高 ,如能抓住其临床特征进行早期诊断和治疗 ,多数患者能恢复健康

A clinical analysis of before 10 years and after 10 years in the old gynecologic diseases

Objective: To study the clinic character of gynecologic diseases and change of their types in old woman during 20 years in order to provide some information about the prevention or treatment of these disease. Methods: 244 cases of old gynecologic in patients and their complications during 20 years from January in 1980 to December in 1999 were analyzed. These cases were compared and divided into two groups before 10 years and after 10 years. Results: The most gynecologic diseases in old women were tumors of genital tract (61 07%), previous traumatic diseases of genital tract (26 64%), non tumor abnormal uterine bleeding (6 15%), genital tract inflammation (4 92%) and other diseases (1 23%). Preparation of malignant tumors was 18 03%, in which ovarian cancer was primacy before 10 years but endometric cancer after 10 years. The morbidity of malignant tumors dramatically decrease after 10 years ( P <0 01). in the concomitant diseases befroe surgery, except hyperlipidemia and pulmonary disease were significantly changed after 10 years group ( P <0 05, P <0 01). Others were no difference between two groups. Conclusions: Although there are many types of gynecologic diseases in old women, most of patients restore their health if they were diagnosed and treated earlier according to their clinic characters.
