高频超声与钼靶X线及二者联合应用对乳腺浸润性导管癌诊断价值的探讨

摘    要:目的探讨高频超声与钼靶X线在乳腺浸润性导管癌中的诊断价值。方法41例乳腺浸润性导管癌患者均行手术并经病理证实,分析彩色多普勒超声声像图特征和钼靶X线结果,比较两种方法单独及联合应用的诊断准确率、误诊率、漏诊率。结果术前超声诊断乳腺癌35例(85.37%),不能定性或良性病变4例(9.76%),无明确异常2例(4.87%);而钼靶诊断结果分别是23例(56.10%),12例(29.27%),6例(14.63%)。高频超声诊断乳癌的准确率高于钼靶X线,漏误诊率低于钼靶,两者联合应用诊断准确率提高至92.68%。结论乳腺浸润性导管癌具有特征性的超声影像学改变,高频超声较乳腺钼靶X线具有更高的准确性;两者联合应用能降低疾病的漏诊率及误诊率,进一步提高诊断准确率。

Abstract:Objective To explore the value of high frequency ultrasound and mammography in diagnosis of breast infiltrative ductal carcinoma. Methods Forty - one patients with breast infiltrative ductal carcinoma were confirmed by pathology and surgery, their sonograms and mammographic results were analyzed, and the diagnosis accurate rate, misdiagnosis rate, missed diagnosis rate in the method of solitude and combination were compared. Results There were 35 cases of breast cancer diagnosed by preoperative uhrasonography ( 85.37% ), 4 cases of indeterminate or benign lesion ( 9.76% ), 2 cases of no definite abnormality(4.87% ) , while there were 23 cases of breast cancer diagnosed by mammography (56.10%) , 12 cases of indeterminate or benign lesion (29.27%) , 6 cases of no definite abnormality ( 14.63 % ) , respectively. The diagnosis accurate rate of uhrasonography was higher than that of mammography, and the misdiagnosis rate, missed diagnosis rate were lower than those of mammography. Combination of two methods increased the diagnosis accurate rate to 92.68%. Conclusion The breast infiltrative ductal carcinoma in high frequency uhrasonography has distinctive imaging changes, it has higher accuracy than mammography. The misdiagnosis rate, missed diagnosis rate can be decreased and the diagnosis accuracy can be increased by combination of both modalities.
