创伤后昏迷患者胃食管反流的临床监测

目的 探讨创伤后昏迷患者胃食管反流的发生情况，为临床治疗护理提供依据。方法应用便携式pH监测仪，将pH电极以pH梯度法定位于患者食管下括约肌以上5cm处，对24例创伤后昏迷患者进行24h食管pH监测，并将监测值与正常值进行比较。结果 24例创伤后昏迷患者中19例占79．2％总评分大于正常值，其中除反流次数与正常值最高限接近外，pH〈4的反流时间占监测总时间的百分比，反流时间〉5min次数，最长反流时间，Boix—Ochoa综合评分各指标均较正常值显著增高。结论创伤昏迷患者胃食管反流多数异常，需加强此类患者预防反流及误吸的治疗与护理。

Clinical monitoring for injured patients with gastroesophageal reflux

Objective In order to study the condition of gastroesophageal reflux for injured patients, and then provide experience for clinical practice. Methods Monitored the pH values among 24 injured patients by handling pH monitor, and then compared the data with normal values. Results The general values in 19 patients were higher than normal values. All the values such as reflux episodes, percentage time in reflux and episodes over 5 minutes were higher than the normal values significantly. Conclusions Gastroesophageal reflux existed in most of injured patients, so related preventive and treating cares should be strengthened.
