286例冠心病病人静息心电图与冠状动脉造影对比分析

目的 探讨静息心电图(ECG)与冠状动脉造影(CAG)诊断冠心病(CAD)的一致性。方法 所有病人均行CAG及ECG检查。结果 286例CAD病人，典型ECG阳性率仅35．66％，与冠状动脉狭窄程度比较，狭窄50％～70％组阳性率12．90％，狭窄702，5～90％组阳性串19．26％，狭窄≥90％组阳性率76．40％；与冠状动脉狭窄支数比较，单支病变组阳性率42．10％，双支病变组阳性率14．68％，三支病变组阳性率66．28％。结论 部分CAD病人没有典型ECG改变，常常引起漏诊。对有典型病史者，应尽早进行CAG检查。

The comparison of normal electrocardiogramand coronary angiography in 286 patients with coronary heart disease

Objective To investigate the concordance between the normal electrocardiogram (ECG) and coronary angiography (CAG) in the diagnosis in patients with coronary heart disease (CHD).Methods 286 patients with CAD were examined by ECG and CAG.Results Of 286 patients, only 35.66% had typical ST-T ischemic changes on electrocardiogram. Compared with the degree of coronary stenosis(CAS), 12.9% positive rate was shown in the patients with 50~70% coronary stenosis, 19.26% in 70-90% CAS group, and 76.40% in 90% group . Compared with the number of coronary artery branch stenosis , there were 42.10 % typical ECG changes in one branch CAS group ,14.68% in two branch CAS groups, and 66.28% in three branches CAS groups.Conclusions Some patients with CHD had no typical ST-T ischemic changes on ECG . Patients with typical chest pain must be examined by CAG as early as possible.
