人工关节置换治疗老年人股骨颈骨折的临床分析

目的探讨人工关节置换治疗老年人股骨颈骨折的临床效果。方法对53例股骨颈骨折患者应用人工关节置换治疗的手术风险、围手术期处理、假体类型和术后关节功能的恢复等情况进行随访分析。结果53例中,除2例脑血管意外后遗症不能早期下床活动外,其余患者在1~4周恢复下地行走,有6例并发肺部感染,经选用有效抗生素和加强护理治愈,无死亡。经随访6个月~5年,按Harris评分标准评定功能:优29例,良15例,可7例,差2例,优良率为83.02%。结论人工关节置换术是治疗老年人股骨颈骨折较理想的方法,能提高患者的生活质量。

Hip replacement arthroplasty in treatment of older patients with fractures of the neck of the femur

Objective To describe clinical outcome of hip replacement arthroplasty in treatment of old patients with fractures of the neck of the femur. Methods 53 patients with fractures of the neck of the femur undergoing hip replacement arthroplasty were retrospectively reviewed for the risk of operation, preoperative management, the prosthesis types and the recovery of postoperative joint function. Results In 53 cases, except 2 patients could not walk early because of the sequel induced by cerebrovascular accident, the others could walk within 1 -4 weeks. Of the 6 cases were associated with lung infection, good response was obtained after treatment with validated antibiotics and delicate care, no death. At the 6 months-5 years follow - up visit Harris Hip Scores were with 83.02% good or excellent results. Of the 29 eases with excellent results, 15 cases with good results, 7 cases with fair results and 2 cases with poor results. Conclusion The hip replacement arthroplasty is a validated therapy for the treatment of old people with fractures of the neck of the femur, which can improve the patients quality- of- life.
