经阴道超声测量足月初产妇宫颈长度评价宫颈成熟度的临床意义

目的:探讨经阴道超声测量宫颈长度评价足月初产妇宫颈成熟度的临床价值。方法:检侧138例足月初产妇宫颈长度、宫颈管宽度、宫颈内口契形区宽度,同时行宫颈Bishop评分。结果:宫颈长度、宫颈管宽度与宫颈Bishop评分显著相关(r=0.58、0.46,P<0.05),且均能预测3天内临产,其中以宫颈长度的预测效果最好。当宫颈长度<30 mm时,预测其在3天内分娩的敏感性、特异性、阳性预测值、阴性预测值为0.82、0.89、0.90和0.82。多因素分析显示,经阴道超声测量宫颈长度,预测宫颈成熟度和分娩时间优于其他指标。结论:经阴道超声测量宫颈长度可用于评价足月初产妇分娩前的宫颈成熟度状况。

Clinical significance of evaluation of cervical maturity by transvaginal ultrasonographic measurements of cervical length among primiparaes

Objective:To assess the clinical significance of evaluation of cervical maturity by transvaginal ultrasonographic measurements of cervical length among primiparaes.Methods:Transvaginal ultrasonographic cervical measurements of cervical length,width and top were performed and Bishop scores were determined in 138 primiparaes.Results:Cervical length and width significantly correlatedwith the Bishop score(r=0.58,0.46,P<0.05),and could predict parturient induction in 3 days.The predictive value of the cervical length was the best.If the cervical length was<30 mm,the sensifvity,specificity,positive and negative predictive value were 0.82,0.89,0.90 and 0.82 respectively in the prediction of delivery in 3 days.According to the statistics,the cervical length measured on transvaginal ultrasonography was the independent predictor for the cervical maturity and parturient induction time,which was better than other indexes.Conclusion:The cervical length measured on can evaluate condition of cervical maturity for the primiparae.
