A型肉毒毒素治疗18个月以下脑瘫患儿的体会

目的报道对A型肉毒毒素（RTX—A）治疗低月龄痉挛型脑瘫患儿足跖屈畸形的体会。方法将48例（共93足）18个月以下足跖屈畸形的痉挛型脑瘫患儿随机分为治疗组和对照组，治疗组采取小腿后肌群多点注射BTX—A结合康复训练，对照组只采用康复训练，分别在治疗前和治疗后第7天做综合痉挛量表评分（CSS）和踝关节背伸角度的疗效评定。结果两组CSS评分和踝关节背伸角度在治疗前和治疗第7天比较，差异均具有统计学意义（P〈0．05），治疗组和对照组在第7天的CSS评分和踝关节背伸角度比较，差异具有统计学意义。结论早期应用BTX—A对低月龄痉挛型脑瘫患儿是有效的、安全的，对促进低月龄痉挛型脑瘫患儿的早期运动发育有积极意义。

Botulinum Toxin Type A in the Treatment of Lower Months Children with Cerebral Palsy.

Objective To report effect of BTX-A which was used to treat lower months children with cerebral palsy. Methods 48 patients were randomly divided into a treatment group and a control group. The 26 petients in the treatment group were treated with BTX-A anr RT,while those in the control group were treated with RT only.The modified eompop site spastieity scale （CSS） and ankle dorsifiexion angle were performed before treatment and at 7 days after treatment. Results The CSS and ankle dorsiflexion angle scores increase significantly in the 2 groups by days 7 of the treatment. The CSS and ankle dorsiflexion angle scores in the treatment group were significantly better than those in the control group by days 7 of the treatment. Conclusion BTX-A injection is effective and safe on lower limb spasticity and motor function in lower month children with cerebral palsy.
