肝硬化患者测定血清脂蛋白及载脂蛋白的意义

目的 探讨血清脂蛋白及载脂蛋白测定在肝硬化疾病中的价值。方法 高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)采用酶法(选择遮蔽),载脂蛋白A1(apoA1)、载脂蛋白B100(apoB100)采用免疫透射比浊法。上述项目全部在O-LYMPUS1000全自动生化分析仪上检测。结果 肝硬化失代偿期血清HDL-C、LDL-C、apoA1、apoB100浓度均明显降低,与正常对照组比较有非常显著性差异(P<0.01-0.001)。肝硬化代偿期血清HDL-C及apoA1降低明显,与正常对照组比有显著性差异(P<0.05-0.01),而LDL-C、apoB100下降不明显,与正常对照组比无显著性差异(P>0.05)。结论 肝硬化患者同时检测HDL-C、LDL-C、apoA1及apoB100水平,不仅可以反映肝脏脂蛋白和载脂蛋白合成代谢水平,同时可以判断肝硬化严重程度及预后,对指导临床治疗有一定意义。

Serum lipoprotein and apolipoprotein levels in patients with liver cirrhosis

Objective We evaluated the serum apolipoprotcin and lipoprotein levels in patients with liver cirrhosis at differentstages of the disease. Methods;Serum HDL-C and LDL-C were measured by enzymatic method. Apolipoproteins were derermined by using immunonephelometric methods. All the items were measured om OLYMPUS1000 clinical chemistry autoanalyzer. Results; Serum levels of HDL - C,LDL- C,apoA1,apoB100 declined significantly in patients with liver cirrhosis out of compensation, compared with normal control(p<0.01-0.001). HDL - C.apoA1 declined significantly in patients with live cirrhosis in compensation stage,compared with normal contral(P<0.05-0.01), While LDL- C,apoBioo were not significantly decreased(P>0. 05) . Conclusion:Serum levels of HDL- C,LDL- C,apoA1 and apoBioo are good markers of lipoprotein and aplipoprotein synthetic metabolism in liver. It also denotes the degree and prognosis of liver cirrhosis,which can be helpful to clinical treatment.
