绝经后脑梗死患者性激素及血脂变化的研究

摘    要:目的 观察绝经后动脉粥样硬化血栓性脑梗死(atherothrombotic cerebral infarction,ATCI)患者血清性激素变化及其对血脂的影响。方法 选择43例绝经1年以上患有ATCI的妇女(A组),平均年龄65.5岁,41名绝经1年以上的健康妇女(B组),平均年龄63岁,以及47名健康育龄行经妇女(C组),平均年龄38岁,进行血清雌二醇、促卵泡刺激素、促黄体生成素及血脂谱测定。结果 A组患者血清雌二醇、高密度脂蛋白胆固醇水平较低,而血清促卵泡刺激素、促黄体生成素、总胆固醇和低密度脂蛋白胆固醇水平则较高,与B组和C组相比差异有显著性意义(均P<0.01)。直线相关分析表明,血清雌二醇与促卵泡刺激素、促黄体生成素、总胆固醇和低密度脂蛋白胆固醇水平呈负相关,而与高密度脂蛋白胆固醇呈正相关。结论 绝经后动脉粥样硬化血栓性脑梗死患者存在严重的性激素失调及脂质代谢异常,其中血清雌二醇水平的降低可能是其发病原因之一。

Abstract:Objective To study the level changes of serum sex hormone and its effect on blood lipid in postmenopausal women with atherothrombotic cerebral infarction (ATCI). Methods Forty three post-menopausal (>1 year) wemen with ATCI (group A, average 65.5 years old), 41 postmenopausal (> 1 year) healthy wemen (group B, average 63 years old) and 47 young healthy premenopausal wemen (group C, average 38 years old) as controls were enrolled to determine serum estradiol (E2), follicle stimulating hormone (FSH), luteinizing hormone (LH), total cholesterol (TC), high density lipoproteincholesterol (HDL-C), and low density lipoproteincholesterol (LDL-C) levels. Results Analysis of variance revealed that serum levels of FSH, LH, LDL-C were significantly higher while E2 and HDL-C were lowering in group A than those in group B and group C (all P<0.01). Correlation analysis showed that serum E2 level was negatively correlated with serum FSH, LH, TC and LDL-C levels, but positively correlated with HDL-C. Conclusion It demonstrates there are significant disturbances of sex hormone and abnormal metabolism of lipid in the patients with postmenopausal ATCI. It is demonstrated in these biochemical changes, the lowering of serum E2 may be one of the pathogenic factors in ATCI.
