老年帕金森病患者血浆同型半胱氨酸水平与认知障碍相关性分析

摘    要:目的分析老年帕金森病(PD)患者血浆同型半胱氨酸(Hcy)水平与认知障碍的关系。方法回顾性分析100例老年PD患者的临床资料,其中伴有认知障碍50例(认知障碍组),认知正常者50例(认知正常组),另抽取同时期的健康体检者50例作为对照(健康对照组)。统计对比认知障碍组和认知正常组患者的基本病历资料,检测比较3组入选对象血浆Hcy、叶酸和维生素B12水平,分析简易智能状况检查(MMSE)评分与血浆Hcy水平等因素的相关性。结果与认知正常组患者相比,认知障碍组患者病程明显延长,而MMSE评分明显降低(P<0.05)。与健康对照组相比,认知障碍组和认知正常组患者血浆Hcy水平升高,差异有统计学意义(P<0.05);认知障碍组患者血浆Hcy水平高于认知正常组,差异有统计学意义(P<0.05)。血浆叶酸和维生素B12水平3组比较差异均无统计学意义(P>0.05)。血浆Hcy水平与MMSE评分存在明显的负相关(r=-0.693,P<0.05)。结论血浆Hcy水平与认知障碍存在显著的相关性,血浆Hcy水平升高是老年PD患者发生认知障碍的重要危险因素。

Abstract:Objective To analyze the correlation between homocysteine( Hcy) levels of plasma and cognitive disorder in elderly patients with Parkinson disease( PD). Methods The clinical data of one hundred PD patients were analyzed retrospectively. Fifty patients had cognitive disorder( cognitive disorder group),and the other fifty patients with normal cognitive function( normal cognition group). At the same period,fifty healthy persons were selected as health control group. The clinical data of cognitive disorder group and normal cognition group were compared. Plasma Hcy,folic acid and vitamin B12levels of the three groups were detected and compared. The correlation between mini-mental state examination( MMSE) score and plasma Hcy level were analyzed. Results Compared with normal cognition group,the course of disease was significantly prolonged, while MMSE scores were significantly lower( P 0. 05). The plasma Hcy levels in the cognitive disorder group and normal cognition group were significantly higher than those in health control group( P 0. 05); the plasma Hcy levels in the cognitive disorder group was significantly higher than that in normal cognition group( P 0. 05). There were no statistic difference of folic acid and vitamin B12in blood plasma among the three groups( P 0. 05). The plasma Hcy levels and MMSE had a negative relationship( r =- 0. 693,P 0. 05). Conclusion The levels of plasma Hcy and cognitive disorder are significantly related,elevated plasma Hcy is an important risk factor of cognitive disorder in elderly patients with PD.
