冠状动脉造影与常规心电图检查诊断价值比较

目的探讨常规心电图在冠心病（CHD）中的诊断价值,指导临床工作。方法在选择性冠状动脉造影（CAG）的400例住院患者中,对常规心电图（ECG）检查进行CHD诊断价值的分析比较。结果 ECG检查的敏感性为78.5%,特异性为51.0%,假阳性率为49.0%,假阴性率为21.5%,阳性预测值为61.6%,阴性预测值为70.3%,准确率为64.8%。心电图检查的阳性检出率随病变支数的增多而增高,冠状动脉狭窄单支病变为65.3%,双支病变为92.2%,三支病变为88.6%。结论 ECG可作为CHD筛选检查的首选方法,具有实用价值,用于发现院外及日常生活中的心肌缺血,是其他检查方法无法替代的。

Diagnostic value of coronary angiography and routine electrocardiogram

Objective To investigate the value of routine electrocardiogram in the diagnosis of coronary artery disease（CHD） .Methods The diagn ostic value of routine electrocardiogram（ECG） was analyzed and compared in 400 cases patients with CHD confirmed by coronary angiography（CAG） .Results The sensitivity of ECG was 78.5%,specificity was 51.0%,false positive rate was 49.0%,false negative rate was 21.5%,positive predictive value was 61.6% and negative predictive value was 70.3%,accuracy rate was 64.8% .ECG positive rate with the increase in the number of diseased vessels increased.The coronary stenosis rate in single vessel was 65.3%,two vessels was 92.2%,three vessels was 88.6% .Conclusion It is suggested that ECG is the first choice for the screening of CHD.It has practical value to examine the patient with CHD outside hospital or in daily life,which can＇t be substitute by other methods.
