本体感觉神经肌肉促进技术在偏瘫下肢康复中的应用

摘    要:目的对脑卒中后偏瘫患者进行本体感觉神经肌肉促进技术(PNF)训练,探讨其对偏瘫下肢康复疗效。方法 60例脑卒中后偏瘫患者随机分为观察组和对照组,每组30例。2组均给予神经内科常规药物治疗,对照组给予针灸、理疗等康复措施,观察组在对照组的基础上应用PNF训练,于康复治疗前后采用Barthel指数评定日常生活活动能力、Fugl-meyer评定法评定2组患侧下肢功能。结果观察组和对照组治疗后2个月Barthel指数评分、Fugl-meyer下肢功能评分均较治疗前提高(P<0.01);观察组和对照组治疗前后Barthel指数评分和Fugl-meyer下肢功能评分评分差值比较差别有统计学意义(P<0.05)。结论合理的利用PNF模式及拮抗肌逆转等技术,有利于降低痉挛、促进分离动作的出现,提高了下肢的运动功能。

Abstract:Objective To investigate the effect of proprioceptive neuromuscular facilitation( PNF) in the rehabilitation of patients with lower extremity hemiparalysis after cerebral apoplexy. Methods Sixty hemiplegic patients after cerebral apoplexy were randomly divided into observation group and control group,thirty patients in each group. The patients of both groups were treated with routine drug treatment. The control group were given acupuncture,physical treatment and other rehabilitation meas-ures. Based on the control group,observation group was added PNF. Barther index was used to assess the activities of daily living and the Fugl-meyer was used to assess the function of lower extremity between two group before and after rehabilitative treatment. Results Two months after treatment of observation group and control group,both Barthel index scores and Fugl-meyer scores were higher compared with before treatment ( P < 0. 01 ). There were significant differences of Fugl-meyer and Barthel index scores difference before and after rehabilitation between the observation group and control group( P < 0. 05). Conclusion Reasonable utilization of PNF and antagonistic muscle retroconversion techniques can improve the motor function of lower extremity by degrading cramp and promoting segregation behaviors.
