血清HE4及CA125联合检测对卵巢癌的诊断价值

目的 探讨血清人附睾分泌蛋白4（HE4）及糖类抗原125（CA125）联合检测在上皮性卵巢癌诊断中的应用价值.方法 比较我院2008年1月-2013年5月妇科肿瘤病房收治的50例卵巢恶性肿瘤和53例卵巢良性肿瘤患者共103份血清中的CA125和HE4水平,同时选取同期本院女性健康体检者50例作为健康对照组进行比较.结果 卵巢恶性肿瘤组患者血清HE4和CA125水平显著高于卵巢良性病变组和健康对照组患者,差异有统计学意义（P＜0.05）;而健康对照组和卵巢良性病变组比较,差异无统计学意义（P＞0.05）.Ⅰ-Ⅱ期卵巢癌中HE4和CA125的阳性率大致一致（P＞0.05）,Ⅲ-Ⅳ期卵巢癌中HE4的阳性率均高于CA125,联合检测CA125和HE4的阳性率高于单独检测HE4及CA125的阳性率,差异有统计学意义（x2=7.846,P＜0.05）.结论 HE4和CA125联合检测能进一步提高诊断能力,在卵巢癌早期诊断中的应用价值值得关注.

Clinical value of combined detection of serum HE4 and CA125 in diagnosis of ovarian cancer

Objective To investigate the application values of the combined detection of serum human epididymis protein 4 （HE4） and carbohydrate antigen 125 （CA125） in the diagnosis of epithelial ovarian cancer.Methods The serum CA125 and HE4 levels in 103 cases including 50 eases with ovarian cancer and 53 cases with benign ovarian tumor were compared by retrospective analysis.While 50 cases women in the corresponding period were collected as a healthy control group.Results Serum of HE4 and CA125 levels in patients with ovarian cancer were significantly higher than that of benign ovarian disease patients and healthy control group （P ＜0.05）.The difference between the healthy control group and ovarian benign group was not statistically significance （P＞0.05）.The positive rate of serum HE4 was almost consistent with CA125 in stage Ⅰ - Ⅱ of ovarian cancer（P＞0.05）,While significantly higher than CA125 in Ⅲ - Ⅳ of ovarian cancer.The positive rate which combined detection of serum CA125 and HE4 was higher than the rate of detection of HE4 or CA125 alone,the difference was statistically significant （x2 =7.846,P＜0.05）.Conclusion Combination detection of HE4 and CA125 can further improve diagnostic capabilities.
