改良B-Lynch缝合术治疗剖宫产产后出血临床分析

目的探讨改良的B-Lynch缝合术治疗剖宫产术中宫缩乏力性产后出血的临床效果。方法选择我院2008年1月-2010年12月剖宫产术中宫缩乏力性产后出血病例38例,实施改良的B-Lynch缝合术16例为观察组,采用传统方法治疗22例为对照组,观察两组手术时间、术中出血量、输血率等临床指标。结果观察组治疗有效率100%、手术时间（43.78±13.20）min、术中出血量（819.45±293.20）ml、输血率18.75%;对照组治疗有效率90.1%、手术时间（70.16±19.72）min、术中出血量（1256.84±598.34）ml、输血率54.5%;两组比较差异有统计学意义（P〈0.05）。结论改良的B-Lynch缝合术操作简单、安全,止血迅速,并能保留生育功能,是治疗剖宫产术中发生宫缩乏力性出血的有效方法。

Clinical Observation of the modified B-Lynch suture technique in the treatment of postpartum hemorrhage of cesarean section

Objective To investigate the efficacy of the modified B-Lynch suture technique in the treatment of postpartum hemorrhage of cesarean section caused by uterine inertia.Methods 38 cases of postpartum hemorrhage of cesarean section from 2008.1 to 2010.12 were divided into two groups.The research group(n=16) was treated with the modified B-Lynch suture technique.The control group(n=22) was treated with traditional technique for postpartum hemorrhagkne of cesarean section.The operation time,hemorrhage volume,puerperal morbidity and therapeutic efficacy between the two groups were compared.Results All cases in the research group were effectively stanched with preservation of uterine and in the control group the effective rate was 90.1%.The operation time in the research group and control group were(43.78±13.20)minutes and(70.16±19.72)minutes respectively.The hemorrhage volume in the research group and control group were(819.45±293.20)ml and(1256.84±598.34)ml respectively.The hemorrhage volume in the research group and control group were(819.45±293.20)ml and(1256.84±598.34)ml respectively.The blood transfusion rate in the research group and control group were 18.75% and 54.5% respectively.The differences were significant for the research group and the control group(P <0.05).Conclusion The modified B-Lynch suture technique is useful for its simplicity of application,life saving potential,relative safety and capacity for preserving the uterus and subsequent fertility.It is an effective treatment in the treatment of postpartum hemorrhage of cesarean section caused by uterine inertia.
