老年高血压患者胆红素水平的变化与心功能的关系

目的研究老年高血压患者胆红素水平的变化与心功能的关系。方法入选住院的老年患者644例,将465例高血压患者作为高血压组;179例无高血压患者作为非高血压组。记录所有研究对象基本资料,检测血胆红素、血脂、肝功,同时进行心脏超声检测,并进行相关分析。结果高血压组患者总胆红素、直接胆红素及间接胆红素水平均显著低于非高血压组(P<0.01)。总胆红素是高血压的一个独立的保护因素;而左心室舒张末容积是高血压独立的危险因素。总胆红素、间接胆红素与B型钠尿肽和C反应蛋白水平均呈负相关。间接胆红素与室间隔和左心室后壁厚度呈负相关,与左心室缩短率呈正相关。结论在老年人群中,生理范围内的胆红素轻度升高是高血压的一个独立的保护因素,同时它也是高血压人群心功能的一个保护因素。

Investigation on bilirubin levels and its correlation with cardiac function in elderly hypertensive patients

Objective To investigate the bilirubin levels in elderly patients with hypertension and to study the correlation between bilirubin levels and cardiac function in hypertensive patients. Methods 465 hypertensive patients were selected as the hypertensive group and 179 non-hypertensive patients were enrolled as the control group.Their basic characters,serum bilirubin levels, lipid levels,hepatic enzyme levels and ultracardiogram data were all collected.Results In hypertensive group,the levels of total bilirubin(TB),indirect bilirubin(IB) and direct bilirubin were all significantly lower than in control group.In the multivariate logistic regression anslysis,TB was an independent protective factor for hypertension,and end-diastolic volume was an independent risk factor.TB and IB were both positively related to brain natriuretic peptide and C-reactive protein levels.IB was also negtively relevant to interventricular septal thickness,posterior left ventricular thickness,and positively relevant to left ventricular shortening fraction.Conclusions In elderly people,the slightly elevated serum bilirubin level within the normal range was an independent protective factor for hypertension,and also a protective factor for ca-rdiac function in hypertensive patients.
