血清胱抑素与高血压患者肱踝动脉脉搏波传导速度的关联研究

目的探讨高血压患者血清胱抑素(CystatinC,CysC)与肱踝动脉脉搏波传导速度(brachial-ankle pulse wave velocity,baPWV)的关系。方法入选986名原发性高血压患者,根据2010年中国高血压防治指南高血压诊断及危险分层标准将高血压患者分为四组:低危组、中危组、高危组、很高危组。对所有入组患者进行血清CysC检测。并应用全自动动脉硬化测定仪,测定其双侧baPWV,取双侧baPWV平均值,分析高血压患者血清CysC与baPWV的关系。结果极高危组、高危组高血压患者CysC水平明显高于中危和低危高血压患者(P<0.05);极高危组、高危高血压患者baPWV水平明显高于中危和低危高血压患者(AllP<0.05)。线性相关分析表明高血压患者血清CysC与baPWV呈正相关性(r=0.381P<0.01)。进一步应用多元线性回归分析调整了年龄、性别、体重指数、收缩压、舒张压、血糖、血清胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、甘油三酯等心血管危险因素的影响后,血清CysC与baPWV仍相关(标化的相关系数β=0.165P<0.01)。结论高血压患者随着危险分层的加重,血清CysC、baPWV进行性升高,而且高血压患者血清CysC水平与baPWV显著相关。

Association between Cystatin C and brachial-ankle pulse wave velocity in hypertensive patients

Objective To evaluate the association between plasma Cystatin C and brachial-ankle pulse wave velocity in hypertensive patients. Methods According the guidelines for management of hypertension, A total 986 untreated hypertensive patients s： were divided by four groups： low risk group（n-210）, moderate risk group（n 286）, high risk group（n=292）, very high risk group（n= 198）. All subjects were examined by the Auto-survey atherosclerosis to measure brachial-ankle pulse wave velocity （baPWV）. Auto-survey atherosclerosis was applied to measure brachial-ankle pulse wave velocity （baPWV）. Serum Cys C was also measured in all hypertensive patients. The associations of Cys C and baPWV were evaluated, Results We tbund that the level of Cys C and baPWV aggravated progressively increased as the risk stratification in hypertensive patients. The increased of plasma Cys C levels was related to baPWV in hypertensive patients （P〈0.001）. After adjustment for traditional covariates, Cys C was independently related to baPWV （R2=0.31; β=0.165 P〈0.01）. Coclusion Our funding indicated that the level of Cys C is associated with baPWV in hypertensive patients.
