口腔卫生综合预防措施对固定矫治中牙周健康的影响

目的：寻求改善固定矫治器治疗中患者的牙周卫生状况的预防措施。方法：选择60例接受固定矫治的正畸患者,随机分为3组：对照组常规交待矫治注意事项;试验A组常规交待矫治注意事项,强调口腔卫生,要求使用改良Bass法刷牙;试验B组在试验A组的基础上,矫治前及矫治后每6个月接受一次龈上洁治。临床检测指标为16、13、21、31、43、46指数牙的龈沟出血指数（SBI）和牙龈探诊深度（PD）。采用SPSS11.0软件对数据进行单因素方差分析。结果：①3组患者SBI治疗6个月时均显著增高（P〈0.05）。组间有显著性差异,对照组〉试验A组〉试验B组（P〈0.05）。②3组患者PD随着治疗时间增加而增大,治疗6个月时对照组与两个试验组间有显著性差异（P〈0.05）,两个试验组之间差异不显著（P〉0.05）。③治疗12个月时,试验A组和试验B组间PD有显著性差异（P〈0.05）。结论：①通过刷牙可以部分改善牙周卫生状况,②定期进行牙周洁治可以明显减轻牙龈炎症,较好地维护口腔卫生。

Effect of oral hygiene measure on periodontal status in patients with fixed orthodontic appliance

Objective：The aim of this study is to find a way to improve periodontal status of patients with fixed orthodontic appliance.Method：60 patients aged from 12 to 17 were divided into 3 groups as the control group,experimental group A and experimental group B.The control group were told the way as routine.Experimental group A were emphasized on the daily oral hygiene,and brushed their teeth with BASS method.Experimental group B were emphasized on the daily oral hygiene,brushed their teeth with BASS method and accepted ultrasonic scaling every six months.A follow-up examination of 6 appointed teeth was carried out at the beginning,after 6 months and 12 months respectively.The periodontal status was scored by 2 parameters：sulcus bleeding index（SBI）and probing depth（PD）.The data of the three groups were analyzed with SPSS 11.0 software package for one-way ANOVA.Result：①SBI of 3 groups increased at time point 6 months significantly（P〈0.05）.Difference was significant among 3 groups at time point 6 months and 12 months（P〈0.05）.②PD of 3 groups increased along with the treatment period.At the time point 6 months,the difference between the control group and two experimental groups was significan（tP〈0.05）while the difference between experimental group A and experimental group B was not significant（P〉0.05）.③PD of experimental group B was significantly lower than experimental group A at time point 12 months.Conclusion：①Brushing teeth carefully everyday can improve the periodontal status of patients with fixed orthodontic appliance.②Brushing teeth carefully everyday plus ultrasonic scaling every 6 months can promote the periodontal status more effectively.
