急性脑血管病病人医院内肺炎纤支镜灌洗疗效评价

目的;评价纤维支气管镜（纤支镜）支气管肺泡灌洗（BAL）对急性脑出血管病（ACVD）病人医院获得性肺炎（HAP）的疗效。方法：150例ACVD－HAP病例随机分为常规治疗组及灌洗治疗组,参照“卒中病人临床神经功能缺损程度评分标准”对两组病人入院时及病程28d先后评分、两组间的死亡、恶化及康复病例数进行分析、对比。结果：死亡及恶化病例灌洗治疗组为34．7％、常规治疗组为73．4％,两组间差异有非常显著意义（P＜0．01）。康复病例灌洗治疗组为65．3％、常规治疗组为26．6％,差异有非常显著意义（P＜0．01）。结论：对ACVD－HAP病人行纤支镜BAL,有利于通畅呼吸道,明确病原菌及指导正确选用抗生素,迅速控制HAP,促进脑损害的康复,达到减少ACVD病死率的效果。

The therapeutic study of broncho-alveolar lavage on the treatment of hospital acquired pneumonia after acute cerebral vascular disorder

Objective To study the therapeutic effect of broncho alveolar lavage(BAL) on the treatment of hospital acquired pneumonia(HAP) after acute cerebral vascular disorder(ACVD).Methods Randomly divide 150 cases of ACVD HAP into two groups:routine treatment and BAL treatment.According to "The scoring standard of clinical neural function deletion of cerebral vascular disorder",score the two groups of patients separately in the patient acquired into the ward and on the 28th day in the ward course.Analyze the cases of death,worse and recovery,and compare the significance of statistic difference in two groups.Results The cases of death and worse in BAL group was 34.7%,and 73.4% in routine group and statistic difference was found between two groups(P<0.01).Recovery percentage is 65.3% in BAL group,26.6% in routine group,and the difference is also very significant(P<0.01).Conclusion To treat the ACVD HAP with BAL,it is good to free the respiratory tract,to clear the pathogens in order to choose antibiotics correctly,to control HAP rapidly,to promote the recovery of cerebral impairment,finally to achieve the objective of reducing the incidence of ACVD.
