硬膜外预先注射消炎镇痛液联合胶原酶治疗颈椎间盘突出症临床观察

目的观察硬膜外预先注射消炎镇痛液联合胶原酶治疗颈椎间盘突出症临床疗效。方法 80例颈椎间盘突出症患者经过针灸、按摩、牵引、理疗等保守治疗效果不佳者分为二组,每组40例。A组：单纯C臂下颈硬膜外腔侧隐窝进路前间隙注射胶原酶。B组：颈硬膜外预先一周注射消炎镇痛复合液联合C臂下侧隐窝进路前间隙注射胶原酶组。观察治疗后两组各个时期的疼痛视觉模拟评分（VAS）以及临床效果。结果治疗后疼痛VAS评分与术前相比显著下降。复合液＋胶原酶组在治疗后2周、1月、2月、3月的VAS评分较单纯胶原酶组明显下降（P〈0.05）。结论颈硬膜外预先注射消炎镇痛复合液联合C臂下颈硬膜外腔侧隐窝进路前间隙注射胶原酶治疗颈椎间盘突出症临床疗效提高。

Clinical observation of epidural analgesic and collagenase on herniation of cervical disc

Objective To observe the effect of epidural analgesia liquid combined with collagenase on herniation of cervical disc. Methods 80 patients with herniation of cervical disc who were treated with acupuncture,massage,traction,physical therapy and other conservative treatment and had no obvious effect, were divided into two groups,40 cases in each group. Group A：C arm under cervical epidural lateral recess approach clearance before injection of collagenase group. Group B： cervical epidural analgesic anti-inflammatory composite liquid combined with C arm side recess approach clearance before injection of collagenase group. Visual analog scale （VAS） of pain and clinical effects were observed after treatment in two groups. Results After treatment（2 weeks, 1 month, 2 months, 3months）, VAS score of pain compared with preoperation decreased significantly.2 weeks,l month,2 months,3months after treatment,VAS scores of composite liquid with collage- nase group decreased significantly compared with collagenase group （P〈0.05）. Conclusion Cervical epidural analgesic anti-inflammatory composite liquid combined with C arm under cervical epidural lateral recess approach of space in front of the collagenase injection in the treatment of herniation of cervical disc has good effect.
