支气管肺类癌的诊断和外科治疗

目的探讨支气管肺类癌的临床病理特征、诊断、外科治疗,以提高临床诊治水平。方法回顾性分析我院1980～2008年手术切除并经病理证实的23例支气管类癌患者的相关临床资料,并结合文献进行分析。结果23例中男12例,女11例;年龄18～78岁,平均42.7岁;发病至就诊时间为1周～12年,平均15.3个月。临床症状包括咳嗽、咳痰、咯血或痰血、胸痛、气促、胸闷、喘息及发热;1例并发异位ACTH综合征。影像学检查显示中央型6例,周围型17例;所有患者接受手术治疗,根治性切除率为91.3%。术后病理检查证实为典型支气管肺类癌18例,非典型支气管肺类癌5例。结论支气管肺类癌缺乏X线特征,难与肺癌和肺部其它肿瘤性病变相鉴别。手术切除是治疗支气管肺类癌最有效的方法。此病预后较好,其中典型类癌预后好于不典型类癌,放化疗作用不确定。

Diagnosis and surgical treatment of bronchopulmonary carcinoid tumors
