部分脾动脉栓塞在外伤性脾破裂的临床应用

目的探讨部分脾动脉栓塞术（Partial Splenie Enbolization，PSE）在治疗外伤性脾破裂的临床疗效。方法11例Ⅲ级以下外伤性脾破裂患者，采用Seldinge法经脾动脉造影后行明胶海绵颗粒部分性栓塞止血。右下腹麦氏点经皮穿刺放置导管引流腹腔积液。结果部分脾动脉栓塞术（PSE）对11例脾破裂患者一次性止血栓塞成功，平均手术时间30～40min，患者术后24～48h即可进食，引流通畅，技术成功率100％，全部病例治愈出院。结论外伤性脾破裂患者经部分脾动脉栓塞术（PSE）及腹腔引流术治疗疗效确切，创伤小，并发症少，保留了脾脏，维护了机体的免疫机制，具有较高的临床应用价值。

Effect Analysis of Partial Splenic Enbolization for Splenic Rupture

Objective To explore the clinical effects of partial splenic embolization（PSE） on traumatic splenic rupture. Methods Eleven patients with traumatic splenic rupture under Grade III underwent splenic artery angiogram by Seldingers method and PSE with gelatinum sponge. The catheter was laid by percutaneous acupuncture at right McBurney point to drainage abdomi- nal hydrops. Results All operations in 11 cases were successfully in one time, the average operation time was 30 -40 minutes, all patients took foods after 24 -48 hours. The drain lines were kept unobstructed in all cases. The successful rate was 100% ,and all of them were cured. Conclusion The PSE with abdominal drainage has good effects,little wound and few complications for the traumatic splenic rupture, and the spleen should be preserved to maintain the immune mechanism. It had high clinical value.
