脑梗死患者血浆同型半胱氨酸水平与颅内动脉粥样硬化的相关性

摘    要:目的探讨血浆同型半胱氨酸(Hcy)水平与脑梗死患者颅内动脉粥样硬化的相关性,为颅内动脉粥样硬化的预防提供依据。方法选取2013年1月-12月收治的脑梗死患者120例,检测其血浆Hcy水平,通过数字减影血管造影进行全脑血管造影评估颅内血管情况。结果脑梗死患者多存在颅内动脉粥样硬化。颅内大动脉(颈内动脉颅内段、大脑中动脉)粥样硬化患者血浆Hcy水平高于大脑前动脉、前交通动脉及后循环系统粥样硬化患者(P〈0.05)。而且大动脉粥样硬化随着Hcy水平的升高而更严重,差异有统计学意义(P〈0.05)。结论脑梗死患者颅内动脉多存在粥样硬化斑块,血浆Hcy水平与颅内动脉(尤其是颅内大动脉)粥样硬化存在正相关关系,血浆Hcy可能是颅内大动脉粥样病变的主要危险因素之一。

Abstract:Objective To explore the relationship between plasma homocysteine level and intracranial artery atherosclerosis in patients with cerebral infarction. Methods A total of 120 patients with cerebral infarction diagnosed between January and December 2013 were selected. Plasma homocysteine level was analyzed and intracranial artery was detected by DSA. Results Intracranial artery atherosclerosis can be found in most of patients with cerebral infarction. Moreover, Plasma Hcy level of patients with large cerebral artery atherosclerosis was much higher than others (P 〈 0.05). The much higher Plasma Hcy level, the severe intracranial artery atherosclerosis were found in internal carotid artery and cerebral middle artery (P 〈 0.05). Conclusions Intracranial artery atherosclerosis is common in patients with cerebral infarction. Occurrence of intracranial artery atherosclerosis is positively correlated with plasma homocysteine level. Plasma homocysteine level may be a risk factor of intracranial artery atherosclerosis in patients with cerebral infarction.
