网膜囊外剥离技术切除胃全系膜在胃癌根治术中的应用

目的探讨网膜囊外剥离技术行胃全系膜切除在胃癌根治术中的临床价值。方法回顾性分析2006年1月~2010年1月我院收治的100例胃癌患者的临床资料,按手术方式分为两组:观察组43例应用网膜囊外剥离技术行全系膜切除,对照组57例行常规胃癌根治术。结果两组在术后病理TNM分期、住院时间等方面比较差异无统计学意义(P0.05);观察组手术时间长于对照组,术后并发症率低于对照组,差异具有统计学意义(P0.05)。术后随访3年,观察组3年累计生存率、转移率分别为38.8%和4%,均明显优于对照组的16.8%和12%(P0.05)。结论对胃癌患者应用网膜囊外剥离技术进行全系膜切除治疗,疗效显著,有利于降低转移率,提高患者生存概率,值得推广。

The Clinical Value of Extra Omental Bursa Dissection Technique for Stomach Membrane Excision in Radical Gastrectomy of Gastric Cancer

Objective To explore the value of extra-omental bursa dissection technique for stomach membrane excision in radical gastrectomy of gastric cancer. Methods A total of 100 gastric cancer cases ad- mitted from January 2006 to January 2010 were analyzed retrospectively. All patients were divided into two groups ： 43 cases as observation group was applied omentum extracapsular dissection technique to resect the total stomach membrane, and 57 cases as control group was performed traditional radical surgery. Results There was no statistic difference in TNM staging and length of hospital stay between two groups. The operative duration was longer and the complication rate was lower in observation group than those in control group（P 〈0.05）. The 3- year cumulative survival rate and metastasis rate was 36% and 4% respectively in observation group, both were better than 16.8% and 12% respectively in control group （P 〈0.05）. Conclusion The omentum extracap- sular dissection technique is an effective method to stomach membrane excision in radical gastrectomy, and it can decrease the metastasis rate and increase the survival rate to gastric cancer.
