中医临床护理路径在类风湿关节炎针刀镜治疗中的应用研究

【目的】探讨中医临床护理路径在类风湿关节炎针刀镜治疗患者中的应用效果。【方法】选取2014年1月至2015年12月住院并接受针刀镜治疗的类风湿关节炎患者60例,按照随机数字表法将患者随机分为路径组和对照组各30例。路径组采用中医临床护理路径进行护理,对照组采用常规护理。比较2组患者针刀镜治疗后的健康教育知晓率、护理工作满意度、住院费用、不良事件发生率。【结果】(1)在术前宣教、术后调护、疼痛护理、辨证施膳、功能锻炼等健康教育效果方面,路径组均优于对照组,差异均有统计学意义(P0.05或P0.01)。(2)路径组患者的满意度高于对照组(P0.01),住院总费用低于对照组(P0.05)。(3)路径组的不良事件发生率少于对照组,差异有统计学意义(P0.01)。【结论】类风湿关节炎针刀镜治疗患者应用中医临床护理路径能缩短住院天数,减少住院费用,提高健康教育的效果和患者满意度,减少针刀镜治疗后不良事件的发生,提高医疗社会效益。

Application of Traditional Chinese Medicine Clinical Nursing Pathway in Minimally Invasive Acupotomy Treatment for Patients with Rheumatoid Arthritis

Objective To explore the efficacy of traditional Chinese medicine (TCM) clinical nursing pathway in the minimally invasive acupotomy treatment for the patients with rheumatoid arthritis (RA). Methods Sixty RAinpatients accepting minimally invasive acupotomy treatment from January of 2014 to December of 2015 were randomized into observation group and control group, 30 cases in each group. The observation group was given TCM clinical nursing and the control group was given conventional nursing. After the acupotomy treatment , health education awareness, nursing job satisfaction, hospitalization expenses and incidence of adverse events of the two groups were compared. Results(1) Patients of the observation group had good awareness of pre-operation education, post-operation nursing, pain nursing, dieting based on syndrome differentiation, and function exercises than those of the control group, and the differences were significant (P<0.05 or P<0.01), indicating that the observation group achieved better efficiency in health edication.(2) The observation group had higher satisfaction degree(P < 0.01) and less hospitalization expenses(P < 0.05) than the control group.(3) The observation group had lower incidence of adverse events than the control group(P<0.01). Conclusion Application of TCM clinical nursing pathway in RA patients who accept minimally invasive acupotomy treatment can result into the shortening of hospitalization time and fee, enhancing the efficiency of health education and the degree of patients' satisfaction, reducing the incidence of adverse events, and improving the social benefits of the health care.
