哑铃状切口加皮桥整形术治疗环状混合痔的临床研究

目的比较哑铃状切口加皮桥整形术和传统外剥内扎术两种手术方法治疗环状混合痔的临床疗效。方法将56例环状混合痔的患者随机分成两组,治疗组28例,采用哑铃状切口加皮桥整形术治疗;对照组28例,采用传统外剥内扎术治疗。根据两组患者的伤口愈合时间、治愈率、住院天数、术后不良反应及安全性等方面的结果作评价。结果治疗组与对照组在住院天数、伤口愈合时间及术后伤口水肿、疼痛、皮赘残留等方面差异有统计学意义(P0.05),而在术后出血、肛门功能及肛门狭窄等方面,两组差异无统计学意义(P0.05)。结论哑铃状切口加皮桥整形术治疗环状混合痔能明显缩短住院天数及创面愈合时间,减少术后不良反应,远期复发率低,术后肛门功能良好。

Clinical Study of Dumbbell-shaped Incision and Skin Bridge Plastic Surgery in Treatment of Circumferential Mixed Hemorrhoids

Objective To compare the curative effect of dumbbell-shaped incision and skin bridge plastic surgery with traditional surgery in treating circumferential mixed hemorrhoids.Methods 56 cases of circumferential mixed hemorrhoids were randomly divided into two groups:treatment group,28 patients with dumbbell-shaped incision and skin bridge plastic surgery treatment;control group,28 patients were treated with conventional Milligan-treatment.The curative effect was evaluated and safety according to wound healing time,cure rate,hospital day,postoperative adverse events were compared between two groups.Results There were significant statistical differences in hospital day,wound healing time and postoperative wound edema,pain,and skin tag residues between the treatment and control groups(P<0.05).There were no significant differences in the postoperative bleeding,anal function and anal stenosis(P>0.05).Conclusion The dumbbell-shaped incision and skin bridge plastic surgery treatment of circumferential mixed hemorrhoids can significantly shorten the hospital stay and wound healing time,reduce postoperative side effects and long-term recurrence rate.
