海洛因海绵状白质脑病凋亡与磷脂改变研究

目的观察海洛因海绵状白质脑病(Heroin spongiform leucoencephalopathy,HSLE)的枕叶及小脑白质的细胞凋亡和磷脂改变特点。方法4例HSLE及6例正常脑尸检分别进行脱氧核糖核苷酸末端转移酶介导的原位缺口末端标记法(TUNEL染色)和酸性苏木红法,检测小脑及枕叶白质中细胞凋亡和磷脂的改变。结果在深层白质TUNEL阳性细胞的表达明显高于浅层白质(t=43.64,P<0.01)和正常对照。正常脑中小脑及枕叶白质磷脂染色呈深蓝色,灰质呈黄褐色,而HSLE小脑及枕叶的深部白质磷脂染色较正常显著降低。凋亡的发生与磷脂脱失具有平行关系。结论HSLE病变区存在凋亡和磷脂脱失现象,磷脂脱失可能导致凋亡并为HSLE的发病原因之一。

Study of apoptosis and phospholipids changes in heroin spongiform leukoencephalopathy

Objective It is to observe the characteristic of apoptosis and characteristic changes on phospholipids in white matter of occipital lobe and cerebellum in heroin spongiform leukoencephalopathy(HSLE).Methods 4 cases of HSLE and 6 cases of normal corpse were autopsied and researched with the terminal deoxynucleotide transferase-mediated deoxyuridine 5-triphosphate nick-end labeling(TUNEL) method and acid hematein staining.The apoptosis and the changes of phospholipids in white matter of occipital lobe and cerebellum were detected.Results TUNEL positive cells were observed obviously in the deep white matter in HSLE relative to those in the shallow white matter(t=43.64,P<0.01) and the normal control.The phospholipids stain in white matter and gray matter of occipital lobe and cerebellum in normal brains showed dark blue and yellow brown respectively,while the phospholipids stain in white matter of occipital lobe and cerebellum in HSLE showed significantly lower than that in normal brains.Apoptosis has a correlation with the phospholipids loss.Conclusion The phenomena of apoptosis and the phospholipids loss are existed in the deep white matter in HSLE.The phospholipids loss might be involved in the apoptosis and the etiology of HSLE.
