补虚通瘀颗粒联合氨氯地平阿托伐他汀钙片治疗冠心病心绞痛的临床研究

目的探讨补虚通瘀颗粒联合氨氯地平阿托伐他汀钙片治疗冠心病心绞痛的临床疗效。方法选取2016年12月—2017年12月在周至县人民医院就诊的冠心病心绞痛患者114例为研究对象,按照随机数字表法将所有患者分为对照组和治疗组,每组各57例。对照组饭前30 min口服氨氯地平阿托伐他汀钙片,1片/次,1次/d。治疗组在对照组治疗的基础上温水冲服补虚通瘀颗粒,0.5 g/次,3次/d。两组患者均连续治疗3个月。观察两组的临床疗效和心电图疗效,比较两组的心绞痛发作频率和持续时间、心功能指标、血管内皮功能指标。结果治疗后,对照组和治疗组的临床总有效率分别为78.95%、92.98%,两组比较差异有统计学意义(P0.05)。治疗后,对照组和治疗组的心电图疗效总有效率分别为71.93%、87.72%,两组比较差异有统计学意义(P0.05)。治疗后,两组心绞痛发作频率和持续时间明显降低,同组治疗前后比较差异有统计学意义(P0.05);且治疗组这些观察指标明显低于对照组,两组比较差异具有统计学意义(P0.05)。治疗后,两组左室射血分数(LVEF)、心排血量(CO)明显升高,左室舒张末期内径(LVEDD)、舒张早期流速峰值/舒张晚期流速峰值(E/A值)明显降低(P0.05),同组治疗前后比较差异有统计学意义(P0.05);且治疗组心功能指标明显优于对照组,两组比较差异具有统计学意义(P0.05)。治疗后,两组血管内皮生长因子(VEGF)、一氧化氮(NO)水平显著升高,内皮素-1(ET-1)水平显著降低,同组治疗前后比较差异有统计学意义(P0.05);且治疗组血管内皮功能指标明显优于对照组,两组比较差异具有统计学意义(P0.05)。结论补虚通瘀颗粒联合氨氯地平阿托伐他汀钙片治疗冠心病心绞痛具有较好的临床疗效,可缓解心绞痛,改善心功能,调节血管内皮功能指标,具有一定的临床推广应用价值。

Clinical study on Buxu Tongyu Granules combined with Amlodipine Besylate and Atorvastatin Calcium Tablets in treatment of angina pectoris in coronary heart disease

Objective To investigate the clinical effect of Buxu Tongyu Granules combined with Amlodipine Besylate and Atorvastatin Calcium Tablets in treatment of angina pectoris in coronary heart disease. Methods Patients (114 cases) with angina pectoris in coronary heart disease in Zhouzhi County People''s Hospital from December 2016 to December 2017 were randomly divided into control and treatment groups, and each group had 57 cases. Patients in the control group were po administered with Amlodipine Besylate and Atorvastatin Calcium Tablets at 30 min before meals, 1 tablet/time, once daily. Patients in the treatment group were po administered with Buxu Tongyu Granules on the basis of the control group, 0.5 g/time, three times daily. Patients in two groups were treated for 3 months. After treatment, the clinical efficacies and electrocardiogram efficacies were evaluated, the frequency and duration of angina pectoris, cardiac function indexes, vascular endothelial function indexes before and after treatment in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 78.95% and 92.98%, respectively, and there was difference between two groups (P < 0.05). After treatment, the electrocardiogram efficacies in the control and treatment groups were 71.93% and 87.72%, respectively, and there was difference between two groups (P < 0.05). After treatment, the frequency and duration of angina pectoris in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). After treatment, LVEF and CO in two groups were significantly increased, but LVEDD and E/A in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the cardiac function indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). After treatment, the levels of VEGF and NO in two groups were significantly increased, but the levels of ET-1 in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the vascular endothelial function indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Buxu Tongyu Granules combined with Amlodipine Besylate and Atorvastatin Calcium Tablets has clinical curative effect in treatment of angina pectoris in coronary heart disease, can relieve angina pectoris, improve cardiac function, and regulate vascular endothelial function, which has a certain clinical application value.
