急性循环衰竭麻醉术中经静脉推注乌司他丁的临床疗效及影响因素分析

目的分析急性循环衰竭麻醉术中经静脉注射乌司他丁的临床疗效及影响因素。方法选取我院自2008年9月～2010年9月收治的150例急性循环衰竭的病人,按照其入院顺序分成两组,即为观察组A组、对照组B组,每组均为75例病患。A组患者在常规抗休克疗法的基础上,经静脉推注乌司他丁1×105U,溶于2 ml生理盐水中;B组患者采用常规抗休克疗法。结果麻醉术中,出现急性循环衰竭时,A组患者恢复时间正常者68例（90.67%）,B组为47例（62.67%）,A组比B组多28.00%（P〈0.05）。A组无死亡者,B组死亡19例（25.33%）,A组死亡率较B组低（P〈0.05）。A组75例经过乌司他丁治疗的患者中及时应用乌司他丁治疗者的效果优于未及时采用乌司他丁者。结论急性循环衰竭麻醉术中注射乌司他丁纠正急性循环衰竭,功能恢复正常的时间短,效果显著,死亡率低,大大提高了病人的生存率。

Analysis of the Clinical Effects of Ulinastatin for Acute Circulatory Failure and Its Influencing Factors

Objective To discuss and analyze the clinical characters of ulinastatin for acute circulatory failure and its influencing factors.Methods 150 cases of acute circulatory failure who had admitted in our hospital from September 2008 to September 2010 were chosen as study objects,whom were divided into two groups according to their hospital admission time,namely group A as observation group,and group B as control group,75 cases for each.Ulinastatin was injected to the vein in group A when the acute circulatory failure occurred in narcotherapy process,while the group B adopted general anti-shock therapy.Results When acute circulatory failure occurred in narcotherapy process,the number of patients with short recovery time in group A were 28.00% more than group B（P0.05）.Group A＇s mortality rate was 25.33% fewer than group B＇s（P0.05）.Among these 75 cases of patients in group A,there were 7 cases had delayed circulatory recovery.And in the 7 patients,there were 6 cases（85.71%） had longer medicament preparation time,which was 59.14% more than those rapidly restored patients.Conclusion It takes short time for ulinastatin injecting to rectify the acute circulatory failure during the narcotherapy process,and it has significant effect and lower death rate,which can improve the patients＇ survival rate significantly.
