成人急性病毒性心肌炎心率变异性与室性心律失常

目的探讨成人急性病毒性心肌炎患者心率变异性(heart rate variability,HRV)与室性心律失常(ventricul ararrhythmia,VA)的关系。方法采用24h动态心电图检测52例急性病毒性心肌炎患者和38例正常人的HRV时域指标,并根据是否有VA将急性病毒性心肌炎患者分为VA组和非VA组,采用完全随机设计的单因素方差分析进行各组间HRV的比较,单因素Logistic回归分析评价HRV与VA的相关性。结果 VA组、非VA组和对照组的24小时正常RR间期的标准差(standard deviation of NN intervals/24h,SDNN)分别为(85.97±15.65)ms、(115±20.56)ms、(133.21±27.65)ms。VA组与非VA组和对照组比较HRV明显降低(p<0.01),非VA组与对照组比较HRV降低(p<0.05),与HRV的Logistic回归分析中,心律失常与非心律失常优势比OR值为3.62。结论成人急性病毒性心肌炎HRV降低与VA有一定的相关性。

THE RELATIONSHIP BETWEEN ACUTE VIRAL MYOCARDITIS AND HEART RATE VARIABILITY

ObjeetiveTo investigate the relationship between heart rate variability （HRV） and ventricular arrhythmia （VA） in adult patients of acute viral myocarditis. Methods The time - domained markers of HRV in 52 patients with acute viral myocarditis and 38 normal controls were detected by 24 - hour Hoher. 52 patients were divided into VA and non - VA subgroups according to if suffer from VA or not. All their HRV recordings were compared and analyzed by the one - way ANOVA medthod, and use the logistic regression method to evaluate the correlation between HRV and VA. Results Standard deviation of NN intervals/24h（SDNN） in VA, non - VA and control groups were （ 85. 97 ± 15.65 ） ms, （ 115 ± 20.56） ms and （ 133.21± 27. 65 ） ms respectively. HRV in ventricular arrhythmia subgroup was significantly lower than that in non -VA subgroup and control group （p 〈0.01 ）, and HRV in non - VA subgroup was lower than that in control group（p 〈 0.05 ）. In the logistic regression result, the odds ratio （OR） for VA and NVA is 3.62. Conclusion HRV in adult patients of acute viral myocarditis was low, the decline of HRV is correlated with VA.
