视频脑电图监测在脑瘫合并癫痫患儿中的应用

目的探讨视频脑电图监测对脑瘫合并癫痫患儿的应用价值。方法对249例处于癫痫发作间歇期的脑瘫患儿采用视频脑电图仪（Nation7128）进行监测。结果 249例脑瘫合并癫痫患儿中138例（55.42%）为部分性发作,其次为婴儿痉挛症85例（34.14%）、强直–阵挛性发作15例（6.02%）、肌阵挛发作4例（1.61%）、Lannox-Gastaut综合征5例（2.01%）、失神发作2例（0.80%）;痉挛型脑瘫198例,所占比例最高（79.52%）,共济失调型脑瘫5例,所占比例最低（2.01%）。脑瘫合并癫痫患儿发作间歇期的视频脑电图主要表现为局灶或多灶痫样放电。结论在对脑瘫合并癫痫患儿进行抗癫痫治疗的同时,对其发作间歇期进行视频脑电图监测是非常必要的。视频脑电图对脑瘫合并癫痫患儿的预后判断和指导治疗是不可缺少的检查手段。

Practical value of Video-EEG monitoring in children with cerebral palsy and epilepsy simultaneously

Objective To study the practical value of video-EEG monitoring in children with cerebral palsy and epilepsy simultaneously.Methods 249 children with cerebral palsy and epilepsy simultaneously were monitored with interictal video-EEG.Results Among 249 children with cerebral palsy and epilepsy simultaneously,138 cases（55.42%） were partial seizures,85 cases （34.14%） were infantile spasms,15 cases （6.02%） were tonic-clonic seizures,and 4 cases（1.61%） were myoclonic seizures,5 cases（2.01%） were Lannox-Gastaut syndrome,2 cases（0.80%） were absence seizures.Children with spastic cerebral palsy tended to be the most patients with seizures in epilepsy（79.52%）,and children with ataxia cerebral palsy tended to be the least ones（2.01%）.Local epileptiform discharges and multilocal epileptiform discharges were main manifestation of abnormal results of video-EEG monitoring.Conclusion It’s very necessary for children with cerebral palsy and epilepsy simultaneously to be monitored with interictal Video-EEG when they have received antiepileptic therapy.Video-EEG is the indispensable item of the examination for judging their prognosis and directing their therapy.
