"外结内扎"法腹腔镜下阑尾切除术(附10病例)

目的探讨腹腔镜阑尾切除术(LA)的技术改进及手术的可行性。方法分析10例采用电凝法处理阑尾系膜,用7号普通丝线"外结内扎"法结扎阑尾根部,无钛夹腹腔镜切除阑尾的临床资料。结果手术均顺利完成,无中转开腹,手术时间20~75 min,术中出血0~5 ml,术后平均住院4 d,术后无切口感染、腹腔感染、内出血、残端瘘、肠粘连等并发症。结论采用"外结内扎"法妥善处理阑尾残端及系膜,行腹腔镜阑尾切除术,安全可靠,疗效满意。

"Extracorporeal knotting and intracorporeal ligature" technique in laparoscopic appendectomy

Objective To explore the technical improvement and feasibility of laparoscopic appendectomy.Methods The clinical data of 10 cases undertaking laparoscopic appendectomy were retrospectively analyzed.Electrocoagulation was used to deal with mesenterium of appendix and "Extracorporeal knotting and intracorporeal ligature" technique was applied using ordinary 7# silk thread instead of titanium clips to ligate appendix root.Results Ten successful cases were achieved in laparoscopic appendectomy.The operation time was 20~75 minutes,blood loss during operation was 0~5 ml,and the average hospitalization after operation was 4 days.There were no cases that occurred postoperative complications including puncture hole infection,intro-abdominal abscesses,internal hemorrhage,stump fistula of appendix,intestinal adhesion and intestinal obstruction.Conclusion This above-mentioned method is safe,reliable,effective,and economical,which can be publicized as one of the best choices for appendectomy.
