延续护理在老年肿瘤患者居家管理中的应用效果

目的：探讨延续护理在老年肿瘤患者居家管理中的应用效果，为老年肿瘤患者提供一种更适宜的居家管理模式。方法自2012年8月起采用方便取样法选取长沙市某社区208例老年肿瘤患者作为研究对象，采用奥马哈系统制定干预措施，分别在第1次化疗出院前、第1次化疗出院后、第2次化疗出院后、第3次化疗出院后两周随访，从而对化疗之后老年肿瘤患者的居家生活进行评价。结果老年肿瘤患者第1次化疗出院前、第1次化疗出院后、第2次化疗出院后、第3次化疗出院后两周认知评分分别为（2．91±177;0．57），（3．92±177;0．54），（6．58±177;0．56），（7．39±177;0．59）分，差异有统计学意义（F＝351．05，P＝0．001）；行为评分分别为（3．45±177;0．47），（4．30±177;0．48），（6．44±177;0．49），（7．00±177;0．52）分，差异有统计学意义（F＝363．45，P＝0．000）；现状评分分别为（2．64±177;0．49），（4．17±177;0．48），（6．40±177;0．51），（7．20±177;0．53）分，差异有统计学意义（F＝405．07，P＝0．000）。结论延续护理从环境、社会心理、生理和健康行为4个方面影响患者，能有效提高老年肿瘤患者居家生活质量，从而促进和维护患者健康。

Effect of continuing nursing care in home management for elderly cancer patients

Objective To explore the effect of continuing nursing care in home management for elderly cancer patients , so as to provide a more comfortable home management for aged patients with cancer . Methods A total of 208 elderly cancer patients were chosen by convenience sampling from August 2012, and Omaha system was used to make intervention measures .Follow-ups were conducted before and after discharge on the first chemotherapy , after discharge of the second chemotherapy and two weeks after discharge of the third chemotherapy in order to evaluate their home life .Results The cognition score of elderly cancer patients before and after discharge on the first chemotherapy , after discharge of the second chemotherapy and two weeks after discharge of the third chemotherapy was respectively （2.91 ±177;0.57）,（3.92 ±177;0.54）,（6.58 ±177;0.56）,（7.39 ±177; 0.59）, and the difference was statistically significant （F=351.05,P=0.001）.Their behavior score was respectively （3.45 ±177;0.47）,（4.30 ±177;0.48）,（6.44 ±177;0.49）,（7.00 ±177;0.52）with statistically significant difference （F=363.45, P=0.000）.And their score of current situation was respectively （2.64 ±177;0.49）,（4.17 ±177;0.48）,（6.40 ±177;0.51）, （7.20 ±177;0.53） with statistically significant difference （F=405.07,P=0.000）.Conclusions Continuing nursing care affects patients from four aspects including environment , social psychology , physiology and health , which can effectively improve the quality of life in elderly patients with cancer , so as to promote and maintain their health.
