血管活性药物治疗高危新生儿重度窒息的临床疗效

目的探讨血管活性药物(多巴酚丁胺、多巴胺、酚妥拉明)联合治疗高危新生儿重度窒息的临床效果。方法选取2016年1月~2017年1月期间我院收治的高危新生儿重度窒息患者64例,按照随机单盲法分为观察组(n=32)和对照组(n=32),对照组给予综合性治疗,观察组在此基础上采用血管活性药物如多巴酚丁胺、多巴胺、酚妥拉明治疗,通过观察血气指标、其他脏器损伤等评估临床效果。结果治疗后观察组患儿p H值、Sa O2、Pa O2高于对照组,Pa CO2低于对照组(P0.05);观察组患儿胃肠功能障碍、心功能损害、肾功能损害、HIE发生率均低于对照组(P0.05)。结论多巴酚丁胺、多巴胺、酚妥拉明等血管活性药物联合治疗高危新生儿重度窒息患者,有利于改善血气指标,减少对其他脏器的损害,降低死亡风险,可在临床积极推广和应用。

Clinical efficacy of vasoactive drugs in the treatment of severe asphyxia in high risk neonates

Objective To study the clinical effect of vasoactive drugs(dobutamine,dopamine and phentolamine)in the combined treatment of severe asphyxia in high risk neonates.Methods 64 high risk neonates with severe asphyxia who were admitted to our hospital from January 2016 to January 2017 were selected and were randomly assigned to the observation group(n=32)and control group(n=32)according to random single-blind method.The control group was given comprehensive treatment,and on the basis of this,the observation group was treated with vasoactive drugs such as dobutamine,dopamine and phentolamine.By observing blood gas indicators and other organ damages,the clinical effect was evaluated.Results After treatment,the pH value,SaO2and PaO2in the observation group were higher than those in the control group,and PaCO2was lower than that in the control group(P<0.05);the incidence rates of gastrointestinal dysfunction,cardiac dysfunction,renal dysfunction and HIE in the observation group were lower than those in the control group(P<0.05).Conclusion Dobutamine,dopamine,phentolamine and other vasoactive drugs in the combined treatment of severe asphyxia in high risk neonates,which is conducive to improving blood gas indicators,reducing damages to other organs,reducing the risk of death,and can be actively promoted and applied in clinical practice.
