PPH加外痔切除术治疗重度环状混合痔在乡镇医院的推广应用

目的对比研究PPH加外痔切除术和传统外剥内扎术的手术效果,探讨PPH加外痔切除术治疗的优势及其注意事项,以便推广。方法 103例择期手术患者分为两组,PPH加外痔切除术组(治疗组)53例,传统外剥内扎术组(对照组)50例,对两组相关指标进行对比分析,包括手术时间、术中出血、平均住院天数、术后疼痛时间、以及术后出血、尿潴留、肛门坠胀、排便困难、肛门狭窄及控便失调、吻合口感染等并发症发生率。结果 PPH加外痔切除术治疗重度环状混合痔患者在改善临床疗效方面优于对照组(P<0.01),且其手术时间、住院天数和术后疼痛时间低于对照组(P<0.01),术中出血、术后出血、肛门狭窄,发生率与对照组相比(P<0.05)。结论 PPH加外痔切除术治疗重度环状混合痔疗效疗好,并发症少,且可通过提高手术操作技巧进一步减少并发症的发生,值得在基层医院推广应用。

PPH and externalhemorrhoidectomy treatment of annular mixed hemorrhoid in township hospital

Objective To explore the treatment of mixed internal ring of the clinical effects of PPH and externalhemor- rhoidectomy compared with the traditional one. Methods A total of 103 patients were divided into two groups： 53 cases un- dergoing procedure for prolapse and hemorrhoid artery ligation and 50 cases undergoing outside bar peeling operation. Preoperative and postoperative variables of two groups were compared. Results Time of surgery, pain, and hospitalization time of PPH group were less than those of control group （P 〈 0. 01 ）. The incidences of intraoperative bleeding, postoperative bleeding, disor- ders anal stenosis and anastomotic infection in PPH group were significantly less than those of control group （P 〈 0. 05）. Con- clusion PPH combined with the treatment of hemorrhoids outside is better than traditional surgical stripping morgan.
