超声测量宫颈长度在早产预测中的价值

目的探讨经腹超声测量宫颈长度预测初产妇发生早产的价值。方法于孕28周、32周超声测量宫颈长度,并随访其孕期经过和妊娠结局。结果①发生先兆早产的134例患者,其中早产59例,早产组宫颈长度短于足月产组宫颈长度,差异有统计学意义（P〈0.01）。宫颈长度与早产发生密切相关,以宫颈长度〈3.0 cm为分界,其敏感度=95.0%,特异度=88.0%,阳性预测值=86.2%,阴性预测值=95.7%。②我院分娩的180例正常足月产患者宫颈长度随孕周改变差异无统计学意义（P〉0.05）。结论经腹部超声测量妊娠期宫颈长度预测早产是安全、可靠、有效的。宫颈长度〈3.0 cm的先兆早产患者应高度警惕早产的发生。宫颈长度〉3.0 cm患者,根据临床表现用药,可以减少保胎药的过度使用。

The predictive value of cervix uteri lenth detected by ultrasonography for premature delivery

Objective Discussing the predictive value of cervix uteri lenth detected by ultrasonography transabdomen for premature delivery.Methods Transabdomen ultrasonography were performed in the pregnant women in 28 and 32 weeks of gestation.Following up their pregnant procedure and outcomes.Results ①59 cases occoured preterm delivery within the 134 cases who occoured threatened preterm labor.The cervix uteri lenth of the preterm delivery women are shorter than that of the women who delivery at term.It has prominent statistic significance（P0.01）.The cervix uteri lenth is closely related to the preterm delivery.As a cervical uteri length less than 3.0cm the cut off value to predict the preterm delivery among women with the threatened preterm labor,the sensitivity is 95.0%,the specificity is 88.0%,the positive predictive value is 86.2%,the negative predictive value is 95.7%.②Of the 180 cases who delivery at term,the cervix uteri lenth is shorter when the gestational weeks is longer.But it is no statistic significance（P0.05）.Conclusion The transabdomen ultrasonography used to measure the cervix uteri lenth is safe,trusty and effective.It should be highly alert that the premature delivery would happen when the cervix uteri lenth of the threatened preterm labor is less than 3.0cm.The tocolytic agent needn＇t be used overmuch when the cervix uteri lenth of the threatened preterm labor is more than 3.0cm.
