声触诊组织量化技术在诊断肾实性结节中的应用

摘    要:
目的:探讨声触诊组织量化技术( virtual touch tissue quantification ,VTQ)在评价肾实性结节硬度中的临床应用价值。方法应用VTQ技术,对57例60个肾实性结节及其肾皮质进行VTQ检测,测量病灶及其周围距病灶1.0 cm以上的肾皮质剪切波速度(shear wave velocity,SWV),并与35例健康志愿者肾皮质SWV进行对比,分别比较病灶间的SWV值、病灶与对应肾皮质的SWV值及病灶组肾皮质与健康志愿者肾皮质SWV值。结果60个肾实性结节包括良性病例27个、恶性病例33个;其中,肾透明细胞癌与肾血管平滑肌脂肪瘤、良性病灶与对应肾皮质间、肾血管平滑肌脂肪瘤与对应肾皮质间SWV值比较差异有统计学意义( P<0.05或P<0.01),而恶性病灶与对应肾皮质、良恶性病灶间以及病灶组肾皮质与健康志愿者肾皮质SWV值比较差异均无统计学意义(P>0.05)。结论 VTQ技术可定量分析肾实性结节的硬度,在肾实性结节的定性诊断中有一定应用价值。

Abstract:
Objective To explore the clinical application value of virtual touch tissue quantification ( VTQ) in the e-valuation of renal solid nodule .Methods VTQ was applied in 57 cases of 60 renal solid nodule and renal cortex to measure the shear wave velocity ( SWV) of the lesions and the surrounding lesions with a distance of more than 1.0 cm. And the SWV was compared with the renal cortical SWV of 35 healthy volunteers .The ratio of SWV between lesion , and the ratio of SWV of the lesion and its renal cortex and of healthy volunteers were compared separately .Results Among 60 renal solid nodule were 27 benign cases and 33 malignant cases .Renal clear cell carcinoma and renal angiomyoli-poma, benign lesions and the corresponding renal cortex , renal angiomyolipoma and renal cortex between corresponding SWV value had significant difference (P〈0.05 or P〈0.01 ),while the malignant lesions and corresponding to the renal cortex, benign and malignant lesions and renal cortex lesion group and healthy volunteers of renal cortical SWV values showed no significant difference (P〉0.05).Conclusion VTQ technique can be used for quantitative analysis of the renal solid nodule and has certain application value in qualitative diagnosis of renal solid nodule .
