不同方法治疗婴幼儿社区获得性肺炎临床研究

摘    要:目的探讨婴幼儿社区获得性肺炎(CAP)不同治疗方法的临床疗效和不良反应。方法100例社区获得性肺炎随机分为阿奇霉素组(A组)50例和头孢呋辛钠组(B组)50例;采用VITEK—AMS微生物分析仪测定病原菌,并用纸片法(Knby—Bauer)没定病原菌的体外药物敏感性。结果A组治疗后痊愈45例(90.0%)明显高于B组39例(78.0%);A组退热时间(3.86±0.95)d低于B组(5.01±1.12)d;A组哕音消失时间(3.81±0.74)d低于B组(5.17±1.22)d,差异均有统计学意义(P〈0.05)。A组阳性株数44株,转阴株数39株,细菌消除率为88.6%;B组阳性株数41株,转阴株数35株,细菌消除率为85.3%;A组3例出现恶心、呕吐,不良反应发生率为6.0%(3/50),B组为8.0%(4/50),差异均无统计学意义(P〉O.05)。结论阿奇霉素是治疗婴幼儿CAP的首选药物。

Clinical Research of Infants with Community-acquired Pneumonia by Different Treatment Methods

Abstract:Objective To investigate the infants with community-acquired pneumonia(CAP)of different treatment methods of clinical effica- cy and adverse reactions. Methods 100 cases of community-acquired pneumonia were randomly divided into azithromycin group(A group) 50 cases and eefuroxime sodium group (B group ) 50 eases ; using VITEK-AMS microbiological analysis to measure the pathogens, and use disk diffusion method (Kirby-Bauer)Determination of pathogenic bacteria in vitro drug sensitivity. Results A group after treatment cured 45 cases (90.0%)was significantly higher than the B group of 39 eases(78.0%)( χ 2=3.98 ,P〈0.05) ;The time in A group defervescence(3.86 + 0.95)days less than the B group(5.01 ± 1.12)days(t=2.416, P〈0.05);The time rales disappearance in group A(3.81 ± 0.74)days less than B group (5.17 ± 1.22)days( t= 2.396, P〈0.05 );The positive in group A of 44 strain, turin negative in 39 strain, bacterial eradication rate was 88.6%; The positive in group B 41 strain,turin negative in 35 strain,bacterial eradication rate was 85.3%,A,B groups of bacteria elimination was no significant difference (χ 2=2-98,P〉0-05) ;The incidence of adverse reactions of nansea,vomifing in group A of 3 eases 6.0%(3/50);B group was 8.0% (4/50)( χ 2= 2.23, P〉 0.05 ). Conclusion The drug of Azithromycin were preferred with treatment in infants and young with CAP.
