结直肠癌患者术后吻合口漏的相关危险因素分析

目的分析结直肠癌患者术后吻合口漏的相关危险因素。方法选取结直肠癌术后吻合口漏患者36例作为发生组,并以1∶1配比选取同期结直肠癌术后无吻合口漏患者36例作为未发生组。收集两组年龄、性别、肿瘤距肛门位置、肿瘤分期等临床信息,进行统计分析。结果肿瘤距肛门位置、肿瘤分期Ⅲ、Ⅳ期、是否合并糖尿病、是否低蛋白、吻合口方式、有无术前新辅助化疗是结直肠癌患者术后发生吻合口漏的影响因素(P<0.05);logistic回归分析显示,肿瘤距肛门位置<7cm、肿瘤Ⅲ、Ⅳ期、糖尿病、低蛋白、手工吻合、术前新辅助化疗是结直肠癌患者术后发生吻合口漏的危险因素(P<0.05)。结论肿瘤距肛门位置<7cm、肿瘤Ⅲ、Ⅳ期、糖尿病、低蛋白、手工吻合、术前新辅助化疗是结直肠癌患者术后发生吻合口漏的危险因素,临床可据此实施干预措施,降低术后吻合口漏发生风险。

Risk Factors of Postoperative Anastomotic Leakage in Patients with Colorectal Cancer

Objective To analyze the risk factors of postoperative anastomotic leakage in patients with colorectal cancer.Methods 36 cases of postoperative anastomotic leakage were selected as the occurrence group,36 patients without anastomotic leakage were selected as non-occurrence group.To collect clinical information of age,sex,location of tumor from anus,stage of tumor,and carry out statistical analysis.Results The location of tumor from anus,stage III,stage IV,diabetes mellitus,low protein,anastomotic pattern and neoadjuvant chemotherapy were the influencing factors of postoperative anastomotic leakage(P<0.05);logistic regression analysis showed,tumor location from the anus<7 cm、tumor stage iii,iv,diabetes,low protein,manual anastomosis,preoperative neoadjuvant chemotherapy were risk factors for postoperative anastomotic leakage in colorectal cancer patients(P<0.05).Conclusion The location of tumor from anus<7 cm、Tumor III,IV,diabetes,low protein,manual anastomosis,preoperative neoadjuvant chemotherapy are the risk factors for postoperative anastomotic leakage in colorectal cancer patients,clinical intervention can be implemented accordingly,and reduce the risk of postoperative anastomotic leakage.
