早期干预对极低出生体重儿远期预后的影响

目的:探讨早期随访干预对极低出生体重儿(VLBWI)远期预后的影响。方法:选取VLBWI 72例,按住院号随机分为对照组和干预组。对照组患儿实施常规养育,干预组实施常规养育加随访干预,并定期对两组患儿进行新生儿神经行为测定(NBNA)和发育商数(DQ)、智能发育指数(MDI)、心理运动发育指数(PDI)的测定。结果:两组NABA评分差异无统计学意义(P>0.05)。12月龄时干预组大运动、精细运动、适应行为、语言和个人-社会性行为5大功能区评分较对照组明显升提高;10,12,24月龄时干预组MDI评分较对照组明显升高;6,10,12,24月龄时干预组PDI评分较对照组明显升高,差异均有统计学意义(P<0.05)。结论:早期随访干预可以明显提高VLBWI的DQ、MDI、PDI,提高VLBWI远期生活质量。

Effects of tarly intervention on the long term prognosis of very low birth weight infants

Objective: To explore the effect of early follow-up and intervention on long term prognosis of very low birth weight infants(VLBWI).Methods: By hospital number,72 cases of VLBWI were randomly divided into control group and interference group.In addition to the conventional breeding for the infants in control group,the interference group given follow-up and intervention.The neonatal behavioral neurological assessment(NBNA),developmental quotients(DQ),mental development index(MDI) and psychomotor development index(PDI) was regularly determined.Results: NABA scores were not significantly different between the two groups(P>0.05).At 12 months of age,the scores of main functional areas(gross motor,fine motor,language and personal-social) were higher in interference group than control group(P<0.05).Compared with the control group,the MDI and PDI scores increased in interference group at 10,12 and 24 months of age,and the PDI scores also increased at 6 months of age(P<0.05).Conclusion: Early follow-up and intervention could improve the DQ,MDI and PDI of VLBWT,and better the long-term quality of life.
