倍他乐克对慢性充血性心力衰竭患者心率变异性的影响

目的　探讨慢性充血性心力衰竭患者应用倍他乐克后心脏自主神经功能的变化。方法　对照组常规抗心衰治疗 ,治疗组在常规抗心衰治疗的基础上加服倍他乐克 ,从小剂量 12 .5mg·d-1开始 ,缓慢增加剂量至 2 5～ 5 0mg·d-1,治疗前和治疗 3个月后分别进行心率变异性 (HRT)分析。结果　与对照组比较 ,治疗组治疗 3个月后HRV指标SDNN、SDANN、rMSSD、PNN50 均明显提高 (P <0 .0 5 ) ,且心功能改善明显优于对照组 (P <0 .0 5 ) ,随访 6个月 ,治疗组的死亡率、再住院率均小于对照组 (P <0 .0 5 )。结论　在常规治疗的基础上 ,加用倍他乐克治疗慢性充血性心力衰竭 ,提高HRV ,增强疗效 ,改善预后。

Effects of betaloc on heart rate variability in patients with chronic congestive heart failure

Objective To investigate the functional changes of cardiac autonomic nervous in patients with chronic congestive heart failure (CCHF) treated by bataloc.Method Patients in the control group received the traditional treatment against heart failure. The patients in therapeutic group received betaloc on the basis of traditional therapy.A initial dosage (12.5 mg·d -1 ),and then increased the dosage gradually to (25～50 mg·d -1 ). The parameters of heart rate variability(HRV) pre-treatment and 3 months after 3 treatment were analized.Results Compared with the control group,these HRV indexes of SDNN,SDANN,rMSSD and PNN_ 50 in therapeutic group all increased significantly at 3 months after treatment ( P <0.05),and the improved cardiac function was apparently superior to the control group ( P <0.05).The rate of mortality and re-hospitalized rate in the treated group were lower than those of the control group at 6th months of followed-up ( P <0.05).Conclusion The betaloc plus the traditional therapy for treatment of CCHF patients can increase the of HRV,enhance the treatment effect and improve prognosis of patients.
