肺部感染性疾病患儿支气管冲洗液细菌培养及药敏结果分析

摘    要:
目的纤维支气管镜采集肺部感染性疾病患儿支气管冲洗液,分析病原菌的分布特点和耐药情况,为临床的诊断与治疗提供实验室依据。方法对2008年10月~2010年9月本院儿科和新生儿科送检的120份支气管冲洗液标本进行细菌培养,以法国梅里埃生产的全自动细菌鉴定药敏系统(VITEK-2 compact)对分离的病原菌做药敏试验和菌种鉴定。结果从73例患儿的支气管冲洗液标本中分离出细菌83株。其中革兰阴性(G-)菌41株,排名前3位为大肠埃希菌(10株)、肺炎克雷伯菌(10株)和铜绿假单胞菌(6株);革兰阳性(G+)菌7株;真菌17株;咽部正常菌群18株。余47例患儿支气管冲洗液无菌生长。药敏试验结果显示G-杆菌对碳青霉烯类、喹诺酮类、氨基糖苷类以及头霉素类抗生素均敏感,对其他类型抗生素耐药性较高。结论小儿肺部感染性疾病支气管冲洗液中病原菌分布以G-杆菌为主。根据药敏结果合理地选用抗生素对病例的治疗和减少细菌耐药性具有重要意义。
