异常踝臂指数与慢性肾脏病患者左室重量指数的关系研究

摘    要:目的通过评估慢性肾脏疾病(CKD)3-5期患者的踝臂指数(ABI)和心脏超声参数,了解二者在CKD中是否存在独立相关性。方法选择2011年1月~2012年12月在上海交通大学附属第六人民医院门诊随诊或住院的接受血液透析前行ABI测定以及心脏超声检查CKD3~5期患者250例为研究对象,根据ABI值将其分为ABI〈0.9组、0.9≤ABI〈1.3组及ABI≥1.3组,对比分析三组患者的一般临床资料和超声结果。结果ABI〈0.9组及ABI≥1.3组左室重量指数(IVMI)(149.8±56.2)、(152.7±53.3)]高于0.9≤ABI〈1.3组(127.8+43.6),差异有高度统计学意义(P〈0.01),且多变量分析显示异常的ABI患者(ABI〈0.9组、ABI≥1.3组)与LVMI的增加独立相关(P〈0.01)。结论CKD3~5期患者中异常的ABI与LVMI的增加独立相关。

Abstract:Objective To assess ankle brachial index (ABI) and echocardiographic parameters in patients with chronic renal disease (CKD) stage 3-5, and to know whether there was an significant correlation between abnormal ABI and echocaroi- graphic parameters. Methods A total of 250 predialysis CKD patients from January 2011 to December 2012 were in cluded in the study. The ABI was measured by an ABI-form device and patients were classified into three groups :ABI 〈 0.9 group, 0.9≤ABI〈 1.3 group, and ABI≥ 1.3 group. Clinical and echocariographic paraments of three groups were compared and analyzed. Results Compared with patients with normal ABI, the values of left ventricular mass index (LVMI) of patients in ABI〈 0.9 group and ABI I〉 1.3 group (149.8±56.2), (152.7±53.3)] were higher than that of 0.9 ≤ ABI〈 1.3 group (127.8±43.6), and then the multivariate analysis showed that, patients with abnormal low and high ABI (ABI〈 0.9 group and ABI≥ 1.3 group) were independently associated with the increasing of LVMI (P 〈 0.01). Conclu- sion Abnormal ABI are independently associated with the increasing of LVMI in patients of CKD stage 3-5.
