全膝关节置换术治疗晚期膝关节炎效果观察

目的观察全膝关节置换术（TKA）治疗晚期膝关节炎（OKA）的临床效果。方法选择2010年8月～2012年10月河南大学附属南石医院骨科采用TKA治疗的晚期OKA患者22例，术后早期进行膝关节功能锻炼．观察患者的临床症状，采用纽约特种外科医院评分（HSS）、美国膝关节协会评分（KSS）、骨关节炎指数（WOMAC）评分及视觉模拟评分法（VAS）进行评分。结果①所有患者均成功完成全膝关节置换，无术后感染、下肢静脉血栓形成，无翻修、假体位置不良、假体松动、进行性病变等并发症发生。②患者的临床症状在术后6个月随访得到明显的缓解，术后6个月KSS评分（功能评分、膝关节评分）、VAS评分、WOMAC评分、HSS评分与术前比较，差异均有高度统计学意义（P〈0．01）。结论晚期OKA患者采用TKA疗效确切，尤其对于老年患者，值得在临床上应用。

Clinical observation on total knee arthroplasty for the end stage knee os- teoarthritis

Objective To evaluate the clinical effect of total knee arthroplasty in treatment of end stage knee os- teoarthritis （OKA）. Methods 22 cases of OKA in Nanshi Hospital Affiliated to He＇nan University from August 2010 to October 2012 were selected, they were all received the total knee arthroplasty therapy. They were taken early functional exercise after operation. The clinical symptoms were observed; and the KSS, VAS, WOMAC, HSS score were recorded. Results ①All the operation were completed successfully, no postoperative infection, deep venous thrombosis, knee re- vised, implant malposition and loosening were occurred; there were no progressive disease and other complications. ②Afler 6 months of operation, the clinical symptoms was partial remission; the KSS, HSS, VAS, WOMAC scores 6 months after the operation were all better than those before the operation, the differences were statistically significant （P 〈 0.01）. Conclusion Total knee arthroplasty is a good choice to the patients with end stage knee arthritis, it has cu- rative effect especially for elderly patients, it is worthy of clinical application.
