16层螺旋CTA对主动脉夹层的诊断价值

摘    要:目的探讨16层螺旋CTA对主动脉夹层动脉瘤的诊断价值。方法对30例主动脉夹层动脉瘤患者行16层螺旋CT薄层容积扫描,对轴位重建图像、多平面图像(MPR)、最大密度图像(MIP)、表面遮盖图像(SSD)、容积再现图像(VRT)进行分析并分型。结果 De Bakey分型:Ⅰ型15例,Ⅱ型7例,Ⅲ型8例。轴位图像、MPR、MIP能清楚准确地显示30例患者病变真、假腔,真、假腔密度差及延时后假腔密度高于真腔的密度变化、撕裂口位置及内膜片形态,显示率达到100%;SSD、VRT较好地显示病变大体外观,VRT显示主动脉与分支血管情况优于SSD。结论16层螺旋CTA对主动脉夹层动脉瘤具有显著诊断价值,可作为主动脉夹层动脉瘤的首选检查。

Abstract:Objective To investigate the value of 16-slice spiral CT angiograph in diagnosis of aortic dissection.Methods Thirty patients with aortic dissection underwent 16-slice spiral CT scanning,and axial image reconstruction,multi-planar reformation(MPR),maximum intensity projection(MIP),surface shaded display(SSD) and volume rendering travel(VRT) were analyzed and classified.Results According to De Bakey classification,dissection types were identified as typeⅠin 15 cases,typeⅡin 7 and type Ⅲ in 8.Axial images,MPR and MIP clearly and accurately showed true lumen,false lumen,density difference between true and false lumen,changes in ratio of density of true and false lumen after delay,location of rupture and intimal flap in 30 patients,and the display rate was 100%.SSD and VRT showed the general appearance of lesions,and the situation of aorta and branch vessels showed by VTR was better than that showed by SSD.Conclusion Sixteen-slice spiral CT angiograph has significant value in diagnosis of aortic dissection and can be used as the first choice for examination of aortic dissection.
