乳腺癌术后辅助化疗开始时间与预后相关性研究

目的研究乳腺癌术后开始化疗的时间与预后的相关性,探索更适当的开始辅助化疗时机。方法回顾性分析接受辅助化疗的303例I-III期乳腺癌患者,分析其无病生存期（DFS）和总生存期（OS）。以手术后开始第1个周期化疗时间为起点时间,按起点时间不同方法分析,阶段时间为界：1～14d化疗组（1组）;15～21d化疗组（2组）;22～28d化疗组（3组）;29～35d化疗组（4组）;≥36d化疗组（5组）。结果无病生存期,在各组中无病生存期整体比较有差别（P=0.024）。各组的5年无病生存期分别为76.0%,85.2%,86.7%,63.8%,64.9%。总生存期整体比较无统计学意义。独立的预后因素中,年龄、肿瘤的大小、腋淋巴结转移数目、化疗药物四个自变量与无病生存相关;X2=11.316,P=0.023。结论乳腺癌术后需要接受术后辅助化疗的患者,术后在≤28d开始辅助化疗有益于无病生存期延长。术后辅助化疗最佳区间22～28d。其中年龄、肿瘤的大小、腋淋巴结转移数目、化疗药物四个自变量与病生存相关。

The research time to start of adjuvant chemotherapy after curative surgery influences survival breast cancer and the prognosis relrvance influence

【Objective】 the research time to start of adjuvant chemotherapy after curative surgery influences survival in early-stage breast cancer and the prognosis relevance influence,Better obtains benefits from the adjuvant chemotherapy.【Methods】 A retrospective review was conducted of 303 patients receiving adjuvant chemotherapy for stage I-II breast cancer between January,1995 to October,2001 at my hospital,The disease-free survival（DFS） and overall survival（OS） were compared among patients grouped by time from definitive curative surgery to start of adjuvant chemotherapy.Accepts the first cyclical chemotherapy time after the surgery as the beginning time,Will start the chemotherapy time to use length time interval dividing line：1~14d（No.1 group）;15~21d（No.2 group）;22~28d（No.3 group）;29~35d（No.4 group）;≥36d（No.5 group）.Median follow-up time was 75 months（range,12 to 258 months）.【Results】 DFS： 5 years not get sick the lifetime whole comparison to have the difference（P = 0.024）.respectively 5 years DFS are： 76.0%,85.2%,86.7%,63.8%,64.9%.OS： P value demonstrated does not have the significance statistics difference.Independent prognosis factor,the age,the tumor size,the lymph node number,the treatment medicine four independent variables to DFS condition to have the function;X2=11.316,P=0.023.【Conclusion】 After our country breast cancer technique needs to accept the patient who the technique assists the chemotherapy,After the technique starts in ≤28 the day to assist the chemotherapy to be beneficial in not gets sick the lifetime extension.time to start of adjuvant chemotherapy after curative surgery for 22-28 day.Age,the tumor size,the lymph node number,the treatment medicine four independent variables to DFS condition to have the function.
