慢性阻塞性肺病患者血清中钠尿肽水平变化和临床意义

摘    要:目的测定早期慢性阻塞性肺疾病(COPD)急性加重期(AECOPD)和稳定期患者血清钠尿肽(BNP)水平的动态变化,以探讨早期COPD对左心功能的影响。方法 60例AECOPD患者分为〈75岁和〉75岁两个年龄组,检测治疗前后血清BNP含量,设60名健康体检者为对照组。结果两组AECOPD患者在治疗前后,与对照组比较,血清BNP含量均有明显升高(P〈0.05),其中高年龄组升高更为显著(P〈0.01),患者治疗前后无统计学差别(P〉0.05)。结论早期COPD患者左心功能即开始受损,且高龄患者更为敏感,急性加重后经积极治疗获得缓解仍不能改善左心功能。

Abstract:【Objective】 By observing the serum level changes of brain natriuretic peptide(BNP) in patients with early chronic obstructive pulmonary disease(COPD) at acute exacerbation phase(AECOPD) and stable phase,to investigate the effect of early COPD on left ventricular function.【Methods】 60 patients with AECOPD were divided into two groups: the younger group(75 years old) and the elder group(75 years old).The serum level of BNP were determined at acute exacerbation phase and stable phase.30 healthy persons were set to control group.【Results】 The serum level of BNP in all patients were significantly higher than that in healthy subjects(P0.05).The change of the elder group was obvious particularly(P0.01).But the reduction of the serum BNP level was no statistical significance after AECOPD patients were treat effectively(P0.05).【Conclusion】 The left ventricular function in patient with early COPD will be impaired and elderly patients are more susceptible.The left ventricular function are not be improved after the treatment of AECOPD.
