人工虫草制剂对老年维持性血液透析患者氧化应激和微炎症的影响

摘    要:
目的 观察口服人工虫草制剂对老年维持性血液透析患者氧化应激及微炎症状态的改善作用.方法 选择2013年2月至10月在本院透析时间超过6个月的40例老年维持性血液透析患者为研究对象,随机分为对照组(20例)和治疗组(20例).对照组给予常规碳酸氢盐血液透析治疗,每周3次,每次治疗4h.治疗组为常规血液透析基础上加用人工虫草制剂口服,0.5克/粒,每次5粒,每日3次.2组均治疗3个月.2组研究对象均于研究开始前及治疗3个月后采晨起空腹静脉血,检测血清超敏C反应蛋白(high sensitivityC-reactive protein,Hs-CRP)、白介素6(interleukin-6,IL-6)、肿瘤坏死因子α(tumor necrosis factor,TNF-α)、过氧化物歧化酶(superoxide dismutase,SOD)、谷胱甘肽过氧化物酶(glutathione peroxidase,GSH-Px)、丙二醛(malondialdehyde,MDA)变化.结果 与治疗前相比,3个月后对照组患者血IL-6、TNF-α、Hs-CRP、MDA水平均显著升高(P<0.01),SOD、GSH-Px显著降低(P<0.01).与治疗前相比,3个月后治疗组IL-6、TNF-α、Hs-CRP、MDA水平均显著降低(P<0.01),SOD、GSH-Px显著升高(P<0.01).2组治疗后,组间比较差异有统计学意义(P<0.01).结论 老年维持性血液透析患者体内普遍存在氧化应激及微炎症,且随着透析时间的延长而加重,而口服人工虫草制剂能改善老年患者的氧化应激及微炎症状态.

Abstract:
Objective To investigate the effects of cordceps sinensis on oxidative stress and micro inflammation state in old patients with maintenance hemodialysis (MHD).Methods In this trial,40 old patients who had received conventional hemodialysis for over half year were enrolled and randomly divided into two groups:control group(treated with conventional hemodialysis,20 cases)and the treated group(treated with conventional hemodialysis plus cordceps sinensis soft capsules,orally,2.5g,twice one day; 20 cases).The course lasted 3 months.There were no significant differences in the basic data(age,gender,weight,duration of hemodialysis,dosage of hemodialysis,use of dialyzer,etc)between two groups before treatment (P>0.05).The levels of high sensitivity C-reactive protein(hsCRP),interleukin-6 (IL-6),tumor necrosis factor (TNF-α),superoxide dismutase (SOD),glutathione peroxidase(GSH-Px),and malondialdehyde(MDA) were tested at the beginning and in the end of the study,and compared between two groups.Results After 3 months of treatment,the levels of HsCRP,IL-6,TNF-α,and MDA in control group were increased (P<0.01),but SOD and GSH-Px dedined (P<0.01) as compared with those in treated group.There was significant difference between two groups (P<0.01).Conclusions Micro-inflammation and oxidative stress exist in old patients with MHD.Cordceps sinensis could improve oxidative stress and micro inflammation state.
