剖宫产术中胎儿胎头高浮娩头方法

目的探讨胎头高浮剖宫产娩头方法。方法2004年10月～2006年3月,在本院行剖宫产的胎头高浮产妇98例,随机分为2组,以改进子宫切口48例为观察组,常规娩头法50例为对照组,比较两组的取头时间、产妇和新生儿并发症发生率。结果观察组取头困难1例,对照组8例,差异有显著性（χ^2=4.14,P〈0.05）。观察组平均娩头时间短于对照组,产妇和新生儿并发症发生率低于对照组,差异有显著性（t=10.38,χ^2=6.58、4.23,P〈0.01、0.05）。结论改进后的娩头法安全、可靠,母儿并发症少。

DELIVERY OF FETAL HEAD IN CESAREAN FOR HIGH-FLOATING HEAD

Objective To study the delivery of fetal head in cesarean for high-floating head. Methods During October 2004 to March 2006, 98 lying-in women with high-floating fetal head underwent cesarean, which were randomized to two groups： observation group （48 cases）, the incision on the uterus was modified; and control group （50 cases）, routine head delivery was carried out. The time of head delivery and complications of the mothers and infants were compared between the two groups. Results Difficult delivery of the head was encountered in one in observation group, and eight in controls （χ^2 = 4.14,P〈0.05）. The time for delivering fetal head was shorter in the observation, and the complications were lower, the differences being significant between the two groups （t= 10. 38;χ^2 = 6. 58,4. 23;P〈0. 01,0. 05）. Conclusion The improved delivery technique is safe, reliable, low maternal and fetal complications.
